CASE MANAGEMENT INSPECTION CONDUCTED IN ARMENIAN

Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management inspection to the above facility on 08/22/23 to amend the complaint report pages 1 & 2, dated 03/01/23. LPA arrived at the facility at 10:47 AM and met with Syuzanna Khudoyan, director who guided analyst on a tour of the facility. During this inspection 35 children were present in the facility with eight staff.

A copy of the amended report was discussed and a copy of the amended report was provided to director, Syuzanna Khudoyan during this inspection.

No citations being issued today.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report was reviewed with the director, Syuzanna Khudoyan at 11:20 a.m..
