Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced Case Management inspection for the purpose for the following up on an unusual incident report. Upon arrival at 10am, LPA met with Facility Representative , Sharon Lee , to whom the purpose of the inspection was explained. Tour of the facility was provided. There were children present during the inspection.

Census was taken. There were 17 children and 4 staff members.

On 10/29/24 an unusual incident report was submitted to the Department regarding parent notifying facility that object was found in C1 nose . C1 was seen by the doctor and object was removed and identified as a little bead.

During the investigation , LPA talked to the C1 parent, who indicated after the pickup two hours later C1 was picking their nose when P1 saw object inside. LPA interviewed 3 staff members present the day of the incident and obtained daily schedule for the class, sign in and out sheets for the day of the incident . The day of the incident 18 children were present with 3 staff members .Information was obtained beads used by the facility as a part of the work time . LPA observed beads used by the facility . Beads come with the cup with specific amount of beads spaces. Per S1 she always counts the beads after the children play to make sure none is missing . Per S2, S3 container has exact number of beads and no extra ones . Per staff members none was missing that day . Per all staff members child did not have an interest to play with beads and was not observed next to the area where beads were stored on the date of the incident . Per all staff members, the day of the incident C1 was behaving as usual and nothing unusual was noticed . Per Director , beads were removed from the class and not accessible to the children.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur. Therefore, no citations issued today.


The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
