On 05/02/2023 Licensing Program Analyst (LPA) Andrew Alemoh, met with licensee, Nella Ghazaryan who guided analyst on a tour of the facility for the One Year Required inspection. There is an extended small building in the back of the house that is used for the day-care. This is a one story 3 bedroom, 3 bathroom home with kitchen, dining room , family room, and garage. Upon arrival LPA observed 11 children in care and 2 adults caring for them which are: Licensee and helper #1. Family members residing in the home include 2 adults (licensee, licensee's husband, and two minor children). Facility hours of operation are Monday - Friday 8AM- 5:30 PM. Incidental Medical Services (IMS) policy was discussed. There is no pool on the premises.

Physical Plant: Main care is provided in the day-care building in the back of the house. Parents drop off their children on the right side of the home which is play yard #1 and the play yard extends to the daycare entrance. Naps are provided on cots in the day-care room. There are cubbies for children to place there belongings. LPA observed a clean and fully stocked refrigerator/freezer in the day-care room. The children use the bathroom located between the learning room and the day-care room. The off-limits areas are the main house including: all three bedrooms, 3 bathrooms and kitchen. Laundry room and garage are also off limits and are kept locked during business hours. The home was inspected inside and out for safety, clean and orderly, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents/cleaning compounds which are kept in the laundry room, and no medicines are on the premises.

Hazardous items (sharp knives are kept in the main house kitchen area in the upper cabinet high enough that they are inaccessible to children) .

Safe and age appropriate toys, play equipment and materials were observed. The smoke detector, carbon monoxide detector and Fire Extinguisher (3A10BC) are all in operable condition. Electrical outlets are inaccessible. No recalled and or prohibited toys or sleep/ play equipment were observed on the premises. There is a designated area for ill children as necessary in the day care area. Per Licensee, no one smokes in the home, and there are no weapon/firearms in the home. The facility sketch is complete and current, there is working telephone (cell).

Fire/Disaster drills are maintained current. Last fire disaster drill was completed on April 2023. There is a fireplace that is located in the main house. LPA observed the fireplace to be screened.

Bathroom: Shower/tub are free of hazards and is made inaccessible to children by being blocked off with a storage cabinet. Toilet and faucet are clean and operable. LPA observed a diaper storage cabinet, as well as a storage closet with blankets and diapers in the children's bathroom. The top cabinet has a safety latch, making the cleaning solutions inaccessible to children.

Kitchen: Sharp utensils, open bottles or alcohol are inaccessible. The home has a clean and fully stocked refrigerator/freezer. Sharp knives are located in the off-limits main house. Kitchen cabinets are storage cabinets for extra paper towels, napkins and dry snacks. Cleaning supplies are in the garage which is locked during business hours. Breakfast, lunch, and PM snack are provided. Licensee stated she currently does have a food program.

Outdoor: There are two play yards that are separated by a baby gate. Play yard #1 is used for the younger children, and play yard #2 is used for the older children. Play yard #1 there is an entrance gate for parents to drop off their children. LPA observed the gate to be locked. LPA observed the water temperature mechanisms to be fully barricaded by an iron fence. For play yard #2, on the left side there is an iron fence that barricades the left side of the home that is off-limits. There are 2 storage sheds in Play yard #2 that have toys, and tools in each side. Per licensee both sheds are locked during day-care hours. The front yard is off limits. The backyard is completely fenced in with a brick wall. There are no pets on the premises. LPA observed age appropriate toys, well secured and safe for children.

There are no bodies of water in the premises.

Advisory/Other: First Aid kit was observed with supplies readily available in the children's bathroom. CPR/First Aid expires 11/30/2023. Mandated Reporter expired. LPA reminded licensee; mandated reporter training/ and CPR must be completed every 2 years.

Documents Provided and or Discussed: Fire Drill Log, Safe Sleep PIN 20-24-CCP and Individual Sleeping Plan (LIC9227). Licensee currently does have child care insurance.

Licensee Nella was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee Nella and discussed the Child Care Licensing Safe Sleep web page at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

An exit interview was conducted, a copy of this report was reviewed and provided to licensee along with the appeal rights.
