25 | THIS IS AN AMENDED REPORT AMENDED REPORT TO REMOVE CITATION DUE TO APPEAL. Licensing Program Analysts, (LPAs) Susann Sanchez and Austin Estrada conducted an unannounced complaint site inspection at the above facility. This report is to cite Case Management deficiencies that resulted from the complaint facility investigation. LPAs Sanchez and Estrada arrived at the facility at 9:30am and met with met with Director Shannon Bastista who gave LPAs a tour of the facility at 9:40am. Administrator, Mona Sangeni arrived around 9:55am.
During today's complaint inspection, LPAs observed 13 children with 1 teacher in the kindergarten classroom. During today's tour, LPAs asked staff how many children are enrolled in each classroom and how many staff are present in each classroom. Staff in room 5 stated there are 25 children enrolled with 2 staff (teachers), and room 8 has 25 children enrolled with 2 staff (teachers). During the course of the complaint investigation, interviews with staff and parents reveled that there are 25 children enrolled in room 8 and can be crowded.
LPA's reviewed absence sheets for the preschool program. Based on absence sheets, room 5 currently has 27 children enrolled and room 8 has 26 children enrolled. Per Administrator Sangani, the facility has children absent daily and if children are over ratio in a classroom, the facility places the child in a different classroom.
Based on today's observation in the Kindergarten classroom there where 13 children with 1 teacher. Once LPAs addressed the ratio, Director placed a child in room 8.
Deficiencies were cited in accordance with California Code of Regulations Title 22. See 809-D. page for deficiencies cited during today's visit in accordance to the California Code of Regulations Title 22, Division 12, Chapter 1.Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.
Exit interview was conducted with Mona Sangani, Administrator, copy of report was given. Appeal rights were issued and discussed. |
