25 | Licensing Program Analysts (LPAs) Alicia Mooberry and Joshua Ortega conducted an unannounced case management inspection to follow up on incidents that were reported to the Department. Upon arrival, LPAs met Director Shannon Batista who provided LPA a tour of the facility inside and outside. All staff have current background clearance at the time of inspection. Licensee Mona Sangani was present during the onset of inspection.
A census was taken and LPAs observed 63 napping children supervised by 3 staff.
Wonderland Preschool self reported two incidents to the department on 10/31/24 reporting the Child #1 sustained an injury requiring medical attention. The facility reported the incidents to the Department within the required 24 hours of being notified by the parent that child received medical care. Child #1 is not longer attending the facility. Incident reported on 11/7/24 will require further follow up.
LPA interviewed staff at facility, conducted observations teacher interactions during nap time and outdoor play time.
LPA Mooberry observed record indicating there were 2 staff and 15 pre-schoolers. Interviews with staff confirmed that children were within ration and supervised during the incident.
Based on the information obtained and LPA observation, and record review, LPA determined there will be no deficiencies cited at this time, per California Code of Regulations Title 22, Division 12, Chapter 1.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. |
