25 | Licensing Program Analysts (LPAs) Susann Sanchez and Jonnisha Culbert conducted an unannounced case management Lead Testing exceedance inspection. LPAs met with Director Avelina Moreno (Abby) who guided LPAs on a tour of the facility. There were 47 children and 8 staff present during the inspection.
On 04/19/23, facility provided a copy of the report that covers the analysis performed on samples submitted from LA Testing on 03/28/23. On 2/15/23, the department received notification from the State Water Resources Control Board (SWRCB), Division of Drinking Water (DDW). The SWRCB report indicated the facility was inspected and samples collected on 2/3/2023 showed a lead exceedance. Faucet reported with Room 1 (faucet B) - 6.61ppb was retested on 3/28/23 and cleared. Documents stated that the LA Testing sampler re-labeled faucet (C) as (A). On 12/23/2202 Room 2 (fuacet C) tested at 5.96ppb and on 2/15/23 tested for 6.53 ppb which is over the required 5.0ppb. On 03/28/23, Room 2 (faucet C) retested at 1.00 ppb. However, LA Testing relabeled faucet as (B).
LPAs collected document titled "Lead Testing (Assembly Bill 2370) Testing History." Document has notes from all three test dates.
LPAs cleared POC dated 03/01/23 during inspection.
Exit interview conducted and report was reviewed with the Director Avelina Moreno. A notice of site visit was given and must remain posted for 30 days.
