25 | Licensing Program Analysts (LPAs) Lilli Babcock and Roxana Lopez conducted a case management inspection, following an Unusual Incident Report (UIR) follow-up investigation conducted on 4/27/22. LPAs met with Teacher Yahaira Gutierrez who guided LPA’s on a tour of the facility. Director, Cindy Sinsun arrived at 9:33am.
LPAs conducted today's inspection for the purpose of a UIR follow up. During the UIR follow up the facility was inspected, and the following was observed.
At 9:15 am LPA’s walked into Room 4 and observed S1 to be out of ratio with 14 children. This poses an immediate risk to the health and safety of children in care. Teacher Yahaira took two children to Room 3 at that time and S1 was then in ratio, Per director- they will submit a written plan on ensuring teacher child ratio, including visual supervision, at all times.
Based on the LPA’s observations and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22.
Deficiencies that are being cited need to be cleared to protect the children’s health and safety.
LPA’s Lilli Babcock and Roxana Lopez informed Director Cindy Sinsun that this report dated 7/26/2022 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
