| Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced case management inspection due to an incident that occurred on 09/12/24. LPA met with Shannon Batista, Center Director, who guided LPA on a tour of the facility. There were 12 children napping and 1 staff present in classroom.
Information reported to the Department indicated that Child #1 was injured while on the playground requiring medical attention, The incident was reported to the Department on 09/13/24, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence.
LPA inspected playground, conducted interviews, reviewed files and obtained documentation during this visit.
Based upon information received from the interviews conducted and documentation obtained it was determined that Child #1 was supervised by Staff #1 during outdoor playtime. Attendance records obtained show that there were 12 children present. The facility was within ratio. Staff #1 is a qualified teacher and had current Pediatric CPR/1st Aid Expires on 8/6/2025.
Playground equipment was inspected for age appropriateness and condition. LPA observed that the playground has equipment and rubber cushioning under the monkey bars are all in good repair.
Based on Interviews, observations and documentation reviewed, there appears to be nothing the facility staff could have done to prevent the incident from occurring.
No deficiencies cited during this inspection.
