An unannounced Complaint Investigation, pertaining to the above allegations, was conducted by Licensing Program Analyst (LPA) Stephanie Li. Risk assessment was made. Upon arrival LPA met with Interim Director Leticia Larios, who guided LPA on a tour of the facility. Census was taken. There are 19 children and 6 teachers present during visit.

LPA conducted interview with Interim Director Leticia Larios and 2 staff. Obtained copies of 6 teacher statements regarding their observations of the incident, copy of staff meeting agenda reviewing classroom supervision, teacher placement, and training sign in, and observed changes to bear classroom center set up.

Based on record review and statements obtained from interviews, no disclosures were made to corroborate the allegation or evidence to support the allegation. Although the allegation may have happened or is valid there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is deemed UNSUBSTANTIATED. *Continued on next page........
