 Licensing Program Analyst (LPA) Cynthia Reyes, conducted an unannounced 1 year annual site Inspection to ensure the health and safety standards as required by the regulations governing child care centers are met. LPA met with Director Nichole Santos and a copy of the Entrance Checklist for Child Care Centers form (LIC 125) was provided. The facility days & hours are Monday-Friday 6:30 AM-6 PM.

LPA began the tour of the facility with Nicole, beginning at the front entrance/office to the facility where the parents will enter and exit from the parking lot. The children sign in/out for the parents is located at the entrance using the electronic system Pro Care. The parent board is located in the same area with all the licensing required postings and are visible for all to see. Facility has telephone service and phone number to the school was verified. Health checks are conducted in each class room. The ill/isolation area is located in the Director office and a couple doors down is the staff rest room that an ill child will use. A mat will be used in the Director office for the ill child to use and rest if needed. Files for staff and children were reviewed.

Infant Care: LPA was guided through the Infant class rooms #1 and 2, where LPA observed, age appropriate Fixtures, furniture and equipment was inspected for age appropriateness and is in good repair, but not limited to cribs, cots, changing tables, and feeding chairs. LPA observed and inspected sleeping equipment to ensure there is a sufficient amount. LPA observed that cribs are free from loose articles and objects. There are no objects hanging above or attached to the side of the crib. Infants 12 months and under should be placed on their back for sleeping. Toys and play equipment were also observed to be age appropriate clean and safe. All surfaces and materials accessible to children, including toys, are toxic free. LPA observed the facility has central heat and air, overhead lights as well as natural light.

LPA observed a bathroom in good condition, water temperature, paper towels, toilet paper, and all were inspected for safety, sanitation and operable. Continue on page 2

Title 22 Regulation Section 101429(a)(2)(b) Responsibility for Providing Care and Supervision for Infants was discussed with the Licensee, including but not limited to documentation that shall be maintained. LIC 9227 (Individual Sleeping Plan) for infants up to 12 months was reviewed. 15 minute sleep checks were reviewed. LPA advised no infants to be swaddled while in care. LPA advised the licensee that infants shall be placed on their backs for sleeping and shall be supervised. Infants shall be checked on every 15 minutes and the time of each 15-minute check shall be documented with child’s name and date. The LIC 9227 Individual Infant Sleeping Plan shall be completed for each infant up to 12 months of age. A copy of the LIC 9227 was provided to Licensee. LPA provided the licensee with a copy of A Child Care Provider’s Guide to Safe Sleep, by American Academy of Pediatrics. LPA also consulted and explained Child Abuse Reporting, Never Shake a Baby, and Safe Sleeping practices

The facility will provide sheets and will wash at the end of each week or as needed. Appropriate Individual storage space was observed to be available for each child and cleanliness was observed in the room. The children will be provided jugs of water and disposable cups and they can bring their own labeled water bottles/Sippy cups. Breakfast, Lunch and PM snack will be provided and prepared by the facility. Menus were observed posted. Containers used to discard food have tight fitting lids. Facility has extra linen and food available if needed.



The facility has a kitchen/food pre area in another area so children do not have access. LPA observed a refrigerator, stove, and microwave and large counter tops for use of food preparation in this area. Space was inspected for safety and cleanliness. The applicant will take measures to keep the facility free of flies, other insects and rodents. All food and drinks are to be labeled with the expiration date visible for all to see.

Carbon monoxide detectors and smoke detectors are present and are in operable condition in the class room. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed. The fire extinguisher type 2A-10BC (Service date 10/25/2022) must be serviced annually or as often as necessary. Fire and earthquake drills must be conducted every 3 months and documented. Last drills were conducted on 05/17/2023. Hazardous items including disinfectants, poisonous, cleaning solutions and any other items that could pose a danger to children were observed to be stored inaccessible to children.

Licensing staff was then guided to the Infant outdoor play area. The yard is self contained by a gate around it for separation from other components on site. Water jugs with disposable cups/water bottles will be used for children while outdoors. LPA observed age appropriate outdoor toys, play equipment and materials to be clean, safe and in good repair, as well as adequate shade available by use of the building and trees. All areas around play equipment and slides are cushioned with material to absorb a fall. LPA advised the licensee that the children need to be within the direct care and supervision, including visual observation and supervision of the teacher(s) at all times. No bodies of water were observed on the premises.

SAFE SLEEP: LPA discussed the safe sleep regulations and discussed the Child Care Licensing Safe Sleep webpage at http://www.cdss.ca.gov/inforescources/child-care-licensing/public-information-and-resoucrces/safe-sleep as an additional resource. LPA also informed applicant/licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at http://www/cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Child care centers shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well being of children, employees and visitors.

Facility has no transportation policy for Infants. Licensee states that any lifesaving medication will be stored in a locked area inaccessible to children in care. First aid supplies were observed to have all the required items including a thermometer. There is a fully equipped first aid kit available in each class room.

Criminal Records clearance: Licensee was reminded that all adults 18 and over including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Lead Testing: Assembly Bill (AB) 2370, Chapter 676, statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water used for drinking and food preparation) for lead contamination before January 1, 2023 and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21,1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21,1-CCP. (Licensee stated testing was completed on 08/19/2022 and was cleared, no proof provided). This facility provides incidental medical services-IMS. LPA reviewed storage of "medication and equipment, supplies, and reviewed children's personnel and administrative records.

Teacher child ratios were discussed in accordance with Title 22 Regulations and observed. Staff names were recorded. LPA discussed that children should be under supervision and observation, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license.

Children’s Records (Infants) were reviewed for completeness; Inspection of required forms were made and documented on the LIC 857. LPA reviewed Staff records, which are documented on the LIC 859. Staff present have proof against TB, measles, pertussis, and influenza, as well as current Mandated reporter. LPA observed during staff file review, that there is at least one person trained in Pediatric CPR and Pediatric First Aid present during this inspection and at all times. Children's roster was reviewed for completeness

***LPA also conducted this inspection as this facility provides services to military families. Director Nichole Santos stated the facility currently does not provides services to any military family.

At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued on this date.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Nichole Santos.
