Licensing Program Analysts (LPAs) Jeanette Estrada and Portia Bowden conducted an unannounced inspection at the facility. LPAs initially met with Lead Teacher Keisha Johnson who allowed LPAs entrance into the facility. Facility Representative, Keokuk Legarde- Early Learning Center Manager, joined at a later time and proceeded with providing LPAs with a tour of the facility. LPAs informed all Facility Representatives that reason for visit was to conduct an annual inspection and to follow up on an incident that was reported to the Department on 10/11/24.
This is a preschool program which consists of three classrooms: Two Early Head Start Classrooms (EHS-B2 and EHS-B3) for ages 2-3 years old and one Head Start (HS-B1) Classroom for ages 3 to 5 years old. Both classrooms are full day programs operating Monday through Friday from 7:45 AM to 2:45 PM. During today's inspection, Classroom EHS-B3 was closed due to low staff but children were placed in the EHS-B2 classroom for the day.
Upon arrival, LPAs observed 8 napping children and two staff in the EHS-B2 classroom and 13 napping children and two staff in the HS-B1 classroom. LPAs observed ratios were in compliance.
All areas identified on the facility sketch were inspected. LPAs observed all documents required to be posted in the classrooms. Furniture and equipment were inspected for age appropriateness and good repair in all classrooms. Telephone service, heating, lighting and ventilation were evaluated and are operable. The facility has central air and heating. Lighting was observed to be in operable condition. Children have access to cubbies in the classrooms to store their belongings. Floors of each classroom were observed to be clean and free of hazards. Children are provided with a napping cot and sheet provided by the program. Per Facility Representative, the bedding is laundered by the program. Per Facility Representative the isolation area is located in EHS 3 and a napping cot is provided if needed.
Each classroom has two restrooms inside the classroom with one toilet and one sink in each. EHS-B2 and EHS-B3 have a diaper changing table. Per Facility Representative, diapering supplies are provided by the program. General sanitation was observed in both restrooms and diaper changing areas.

Availability of indoor drinking water was observed in the classrooms via water containers and disposable cups. Each classroom also has a water fountain that children are allowed access to. Per Facility Representative, Long Beach Unified School District tested all water sources and received a report with no exceedances on 9/11/23.
Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Disinfectants and cleaning solutions are stored in locked cabinets in the EHS-B2 classroom. Cleaning supplies are kept on high shelves out of reach of children in HS-B1 and EHS-B3.
Medications are stored in the emergency backpacks in each classroom. LPAs inspected medication. Medication was in original packaging accompanied by the IMS plan.
Per Facility Representative, there are no poisons stored at the facility. LPAs observed an operable carbon monoxide detector in the facility. The facility is equipped with a centralized fire alarm system throughout the school. Per facility representative there are no firearms stored on the premises. There are no pools or bodies of water at the facility. Children's roster was observed and is current. LPAs observed sign in/out sheets for each classroom. LPAs verified the correct number of children were signed into each classroom. Disaster drills are conducted every month and are documented. The last documented drill was conducted on 10/17/24.
The program provides breakfast, lunch and an afternoon snack. Per Facility Representative all meals are provided by the LBUSD Head Start program. Menus are posted once a month in the classroom where it is visible by the child's authorized representative. Menus are also provided to parents. Menus for the past 30 days are available upon request. Per facility representative, children with allergies are provided alternatives and LPAs observed that allergy lists are maintained in the classrooms.
The outdoor playground area was evaluated. The outdoor area is fully enclosed and is used only by the Head Start Program. Outdoor equipment is in a safe condition, free of sharp, loose or pointed parts. All areas around or under high climbing equipment and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water is provided via the water container and disposable cups in the classroom which are taken out when children play outside. Children’s Records were reviewed for completeness. Children's records contained all required documents and information. Staff records were reviewed. Files reviewed contained the required documents. At least one staff's pediatric first aid/cpr was current ( expires 12/2025). Facility Representative was reminded Mandated Reporter training and pediatric first aid/cpr shall be completed every two years.

During this visit LPAs also followed up on an incident that was reported to the Department on 10/11/24. It was noted on the Unusual Incident Report (UIR) that Child 1 experienced a medical emergency due to a pre-existing condition (seizure) during outside time on 10/11/24. Staff 1 was present outside with Child 1 and assisted them when the medical emergency occurred. Staff 1 was able to catch Child 1 as they were experiencing the medical emergency. Staff 1 then carried Child 1 into the classroom where Staff 2 was assisting two other children with another activity. Staff 2 kept child calm and called other facility staff for assistance. 911 and Child 1's parents were called and both arrived within 10 minutes. Staff 2 followed instructions from 911 operator while waiting for ambulance to arrive. Per Facility Representatives, Staff at the facility were previously informed of child's condition and they were trained on how to assist child on steps to take during an incident. LPA interviewed facility staff that were present during the incident. LPA was informed that they have implemented inquiring with parents about required medication for child during the entrance health check. LPA reviewed the Individualized Health Care Plan that has been created for Child 1 following the plan created by Child 1's neurologist. Child 1 returned to the program today 10/17/24.
Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and
administrative records.
