Licensing Program Analysts (LPAs) Cynthia Reyes and Diana Ortiz conducted an unannounced site inspection to investigate the above complaint allegation. This inspection is to ensure the health and safety standards as required by the regulations governing child care centers are met. LPA met with Lindsey Scothorn - Assistant Director who began the tour of the facility with the LPAs and half way through Director Leticia Alvarado arrived and continued the tour with the LPAs.

During the course of this investigation, LPAs conducted Interviews, reviewed and received records and LPAs own observation.

Physical Plant-Per Reporting Party (RP) she has dropped off her child at the day-care and observed cockroaches near her child's cubby. RP has also observed cockroaches in the preschool classrooms

Per LPAs observations and interviews conducted the facility has issues with pest, mostly stated and observed were cockroaches and ants.

Based on LPAs observations and interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 & Chapter 1, are being cited on the attached LIC 9099D page. Deficiencies that are being cited need to be cleared to protect the children’s health & safe.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100

Also, LPA Reyes informed Leticia Alvarado, Director to provide a copy of this licensing report dated 09/17/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A copy of the LIC 9224 was provided to the director on this date.

Buildings and Grounds: The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. The licensee shall take measures to keep the center free of flies, other insects, and rodents.

Director states already has pest control with Ecolab and will request a reassessment and ask what other steps can be done and adding more continuous check ins to ensure the issue gets solved and look into additional fumigation such as having indoor sprayed and tent over campus if needed.

This requirement is not met as evidenced by: LPAs own observations and interviews conducted, the facility has insects such as cockroaches, ants and spiders. Pest control services are conducted once a month for minor services. This poses an immediate health and safety risk to children in care.

Director states will submit to the Department, full detailed documents from the pest control as to what will be done and will request for pest control to do major services twice a year.
