32 | In regards to allegation # 1 staff did not ensure reporting requirements were followed # 2 staff do not ensure adequate supervision is provided to children in care and # 3 staff do not allow children’s responsible parties to observe classrooms while children are in session. LPA conducted interviews with staff # 1- 7. All staff statements corroborated the process to report incidents is writing up a report- depending on severity of incident parents will be called at that time- once child is picked up report is signed by parents or authorized representative and a copy is given to parents and original stays in the facility. Staff # 4, 5 and 7 disclosed that they were aware of the bruise and scrape on chin. Per staff # 4, 5 and 7 the day before the bruised/scraped chin was brought to their attention, child # 1 was pushed by another child during play time. Staff checked child # 1 and only observed a scrape on the knee- incident report was written on that day and parent was informed. Staff # 4, 5 and 7 corroborated that bruised scraped chin was not added to the incident report since it was not observed when child had incident. Staff # 1-7 corroborated that they have not denied parents or authorize representatives entry to the facility or classroom and parents are allowed to come into classrooms if parents request it.
LPA conducted interviews with parents. Parent’s statements corroborate that they do not have any concerns with supervision in the classroom, they have been informed and received incident reports when their child has been hurt and have not been denied entry to the facility.
This agency has investigated the complaint alleging Staff did not ensure reporting requirements were followed- Staff do not ensure adequate supervision is provided to children in care and Staff do not allow children’s responsible parties to observe classrooms while children are in session. Based upon the evidence as presented above, the allegation has been determined to be Unsubstantiated. A finding of Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove alleged violation(s) did or did not occur, therefore at this time the above allegation is unsubstantiated.
A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with Site Supervisor Kenia Villegas.
