25 | Licensing Program Analyst (LPA) Roxana Lopez conducted a Case Management-Deficiencies visit on this date to address deficiencies revealed during an inspection conducted today.
LPA initially arrived to the facility at 9:00 am- during tour LPA walked into the day care room to take census. LPA observed 11 children and 4 staff in the classroom. 2 staff were sitting down with 11 children, 9 of the children were sitting directly on the carpet while 2 were sitting in low highchairs with trays on- the other 2 staff were doing other things in the classroom. Per staff children in the highchairs were sitting there because "they distract the other children". LPA advised that children should not be restrained and asked for the children to be removed from high chairs- and if extra support was needed the other 2 staff should join circle. Children were removed from high chairs and high chairs were placed in storage room during visit.
Based on LPA observations and records review, the following deficiency listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Exit interview conducted and report was reviewed with the Licensee- Director, Ofelia Aguilar and Site Supervisor Kenia VIllegas.
