At 10:30 am Licensing Program Analysts (LPAs) Roxana Lopez and Saul Valenzuela conducted a Case Management - Licensee Initiated visit for the purpose of evaluating a request for a capacity increase from 8 to 14. LPA met with Licensee Keisha Franklin, who guided analysts on a tour of the facility. There were no children present during this inspection. Per Licensee, there are 4 children currently enrolled. The hours of operation are Monday through Friday from 7:00 AM-6:00 PM. Fire clearance for a capacity of 14 was granted on 03/20/2024. Licensee, stated that her daughter/assistant was in the library with 3 children.

This is a one-story home which consists of 3 bedrooms, 2 bathrooms, kitchen, dining room, living room, garage, and backyard (fenced). LPA observed a fireplace in the living room, which it is barricaded and off limits for children. Per Licensee, the children use the living room, dining room, 1 bathroom and back yard for play. Areas off limits to children and parents include: 3 bedrooms,1 bathroom, kitchen and detached garage. All areas identified on the facility sketch were inspected.

The home was inspected for safety, comfort, cleanliness, telephone service via cell phone and landling, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children. There are age-appropriate toys and equipment on the premises.



The valve on the required 2A 10BC fire extinguisher indicates fully charged and was serviced on 08/22/2023, as indicated on service tag. Smoke and carbon monoxide detectors were tested and are operable on this day.------------------------- pg. 1 of 3 ----------------------------------------

Licensee and assistant has current CPR/First Aid certification, expires on 01/2025. Licensee's mandated Reporter Training Certificate, expires on 10/13/2024.

At 11:45 Licensee stated that her assistant does not have current mandated reporter training or immunizations.

Mandated Reporter Training: H&S 1596.8662: Beginning January 1, 2018, all licensed providers, applicants, directors and employees to complete training as specified on mandated reporter duties. Training is available at: www.mandatedreporterca.com
Immunization Requirement: H&S 1597.622: Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles.

The children will use the back yard for outdoor play, the yard was observed to be fenced. LPA's observed appropriate toys and materials for children to use. LPA's did observed a back house in the backyard separated by a fence. LPA's advised that if the back house has the same address the tenants will need to be fingerprinted and associated to the license.

Children's records were reviewed and observed to be complete

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. ---------------------pg. 2 of 3------------------------------------

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Per licensee no infants are currently enrolled.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee, Keisha Franklin confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
Licensee is seeking to provide care for 14 children 0-11 years old. Based on licensing staff observations, the following corrections need to be corrected prior to obtaining a large family child care license. Corrections are due 4/15/2024. Once corrections are submitted The application will be submitted for final review to the department. Once licensed, the applicant is required to adhere to the terms and limitations stated on the license.

- Assistant's mandated reporter

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Licensee Keisha Franklin.------------------------ pg. 3 of 3 ------------
