13 | On 2/3/2023 at 10:00 a.m., Licensing Program Analyst (LPA) Carolyn Tuba conducted an unannounced complaint inspection to deliver findings of the above allegations. A Covid risk assessment was conducted when entering the facility. LPA met with Director; Wendy Mitchell guided LPA on a tour of the facility. There was a total of 53 children present with 6 staff.
According to Reporting Party (RP), the facility was operating out of ratio in 2019 (exact dates unknown). LPA was unable to determine what classroom(S) were over ratio or obtain additional information as the complaint was anonymous. However, LPA conducted interviews with Staff #1 (S1), #2 (S2), #3 (S3) and #4 (S4). Staff was unable to confirm or deny out of ratio issues in 2019 but stated that recently there may have been times that facility staff operated out of ratio.
Based on the evidence obtained during the course of the investigation through interviews, the evidence does not support, nor disprove the above allegation that facility staff operated out of ratio. Therefore, the allegation has been determined to be unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted, appeal rights and report were give to Director, Wendy Mitchell.
