NARRATIVE

On 02/19/25,1:02 pm Licensing Program Analyst (LPA) Shushanik Safaryan conducted an annual random inspection to the above facility. LPA met with Gayane Chilingaryan , Licensee to whom the reason for the visit was explained. Per Licensee, there are 2 infants currently enrolled. There were 2 children present upon arrival. The licensee was observed to be operating within the license capacity limitations. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. Per applicant operation hours are Monday to Friday , 7:30 a.m. to 6:00 p.m. During this visit, all areas identified that are accessible for children to use were inspected for the safety, comfort, and cleanliness. This is a one-story home located on the first level. The home consists of 2 bedrooms, 2 bathrooms, kitchen, dining room, living room (FIREPLACE: inaccessible), den, garage, front yard, and backyard (fenced). The children use the one bedroom, one bathroom located between den and bedroom, den (activity room), living room, dining room and fenced backyard. Per applicant children only pass through the living room and dining room when come from the main entrance. Living room and dining room are not used for day care . Per licensee, areas off limits to children and parents include: 1 bedroom, one bathroom in the hallway, kitchen, front yard and garage. Page 1 of 4

At 1:00 pm , Licensee guided LPA to the tour of the home. LPA was toured starting from the living room, dining room where LPA observed sofas, coffee table, and TV. LPA observed child safety gate that blocks the kitchen and child safety gate from the den to the living room. Next LPA toured den area used for children as an activity space where LPA observed another child safety gate blocking the door to the yard. In the den LPA observed children’s toys , cubbies , table, chairs. Next LPA toured the bathroom available for the children and did not observe any hazards. LPA toured the bedroom used for children . In the bedroom , LPA observed 3 pack n plays and 2 napping infants. Per licensee this room used for napping only. LPA observed door to the licensee`s bathroom and bedroom , which observed to be locked. Next LPA toured the kitchen where observed required 2A 10BC fire extinguisher fully charged with the service date 03/12/2024. Smoke and carbon monoxide detectors observed in the den but were not tested since children were napping .
Next LPA toured the outdoor play area. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA did not observe any objects that can pose a danger to children on the outdoor yard. The licensee states that supervision is always provided.
There is telephone service via a cellphone and landline that stays at the facility during operation hours. There is central air and heating in the home.
Detergents, cleaning compounds, and medications were observed to be inaccessible to children. Per Licensee, there are no poisons in the home. LPA advised that any poisons should be locked under key or combination lock. Licensee states that there are no firearms or weapons stored in the home.
All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Last drill documented was conducted on 12/2024.
The home is observed to be clean and orderly. There are first aid supplies available in the kitchen . There are age-appropriate toys available for children.
Facility License, Emergency Disaster Plan, Parent’s Rights Poster, and Earthquake Preparedness Checklist were observed to be posted in den area.

Per licensee, she does not provide food for children in care. Children bring their own food.
Licensee states she is currently caring for 2 infants only and does not have any assistants . LPA observed a sleep log and LIC 9227 for each infant.
At 1:45 pm , Licensee`s records were reviewed. The licensee has completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 08/2026.
LPA observed that the Licensee does have proof of Mandated Reporter AB 1207 compliant Child Care Training Certificate on file with expiration on 01/06/2026. www.mandatedreporterca.com
Children’s records were reviewed, including emergency information, and were observed to be complete. LPA created an LIC 857, Children’s Record Review and LIC 811, which documents staff and children’s files reviewed during this inspection.
Per licensee, there is only one dog in the home and stays in the off-limit areas. LPA observed dog in the kitchen behind the child safety gate.
Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home.
LPA did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that fall into these categories are not permitted in a family childcare facility.
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA advised the licensee how to access forms, regulations and quarterly updates on the Child Care Licensing website at www.ccld.ca.gov.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California

During the exit interview, the LICENSEE Gayane Chilingarian , confirmed that there are no Registered Sex Offenders living in the facility.

Currently, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days .Exit interview was conducted with the licensee Gayane Chilingaryan on 02/19/2025.
