25 | Licensing Program Analysts (LPAs) Lilli Babcock and Saul Valenzuela conducted an unannounced Case Management follow-up inspection due to an incident that occurred on 4/13/24 and was reported to the Department on 4/15/24. LPAs met with Director, Betty Castillo to whom the reason for the visit was explained. Ms. Castillo guided LPAs on a tour of the facility. Census was taken. There were 7 staff present caring for 53 children. The facility was observed to be operating within the license capacity limitations.
On April 15, 2024, an incident was self reported to the Department by the facility. The facility reported a parent allegation that a child's personal rights were violated while in care.
During the inspection conducted on May 21, 2024, LPAs conducted additional staff interviews regarding the incident with Child #1, and amended the licensing report dated 5/9/24. LPAs attempted to view footage from the facility cameras, however footage was not available. LPAs also obtained documentation including but not limited to staff declarations.
No deficiencies are being cited at this time.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
