Licensing Program Analysts (LPAs) Susann Sanchez and Joshua Ortega conducted an unannounced annual inspection. LPA met with Director Elisa Coburn and Assistant Director Lizbeth Ramirez who guided analyst on a tour of the facility. This is a preschool program which consist of three classrooms; Room 1, Room 2, and Room 3. Facility operation hours are Monday to Friday from 7:30 AM to 5:00 PM. The facility also offers a part time program from 8-11:10am and 12:45pm to 3:55pm .

All areas identified on the Facility Sketch were inspected. Room 1: 18 preschoolers with 3 staff; Room 2: 20 preschoolers with 3 staff; Room 3: 13 preschoolers with 3 staff. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are taken home each week to be washed. Napping equipment (mats) were observed. Per Director, the isolation area is located in the office. Facility also has a designated bathroom if child was to become ill located adjacent to the office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms. Children bring their water containers from home and are filled with a water filtration system if needed. LPA also observed water fountains in each classroom and in the play ground. Director stated that children also take there water bottles outside in addition to the fountain. Water was tested for lead exceedence in February 2023.


Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. LPA observed that poisons are locked in the kitchen. Carbon monoxide detectors were observed in each classroom and are operable.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free. There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

Children’s records were reviewed for Admission Agreement, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, LIC 613-Personal Rights, LIC 702-Health History, and LIC 701 Physicians Report, and Immunizations Records.

Staff records were reviewed for Education, LIC-501: Personnel Record, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, LIC503- Health Screening, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children's roster was reviewed and is current. Sign-In and Sign-Out procedures were reviewed. Parents sign in electronically by using Care Connect. Children present were signed in. Disaster drill log was available, last drill was conducted in 12/9/2024

Food is provided by the facility. The food is delivered daily ready to serve by a vendor called TLC. The facility also provides AM and PM snack. First Aid supplies were observed. Per Director, medication is administered at the facility accompanied by a doctors note.

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.



Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Owner Eliza Coburn.
