25 | ANNUAL AND REQUIRED INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual and required inspection to the above facility on 06/02/23. Licensee has three (3) years probationary license effective 05/29/2020. LPA arrived at the facility at 12:25 PM and met with Haykanush Baroyan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, was Azatuhi Baroyan, Licensee’s Assistant. Per licensee, facility hours are Monday to Friday from 7:30 a.m. to 10:30 p.m.
This is a one story home which consists of 3 bedrooms, 2 and 1/2 bathrooms, kitchen, dining room, living room (FIREPLACE: inaccessible), day care room, converted garage, front yard and backyard (fenced).
The children use the day care room, 1 bedroom, 2 bathrooms, converted garage for limited activity only, and backyard fenced. Per licensee parents enter the facility from the side door and drop off and pick up the children from the kitchen door. Per licensee, areas off limits to children and parents include: 2 bedrooms, living room, dining room, kitchen, front yard, and back of backyard (Pool Area). During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
There is a in ground pool on the premises which is located in the backyard. Pool is inaccessible and surrounded with five feet high fences, with less than 4 inches opening. Also gates swing away form the pool, self closed and have a self-latching device located no more than six inches from the top of the gate. LPA observed the gate was locked with a key lock. The fence meets Title 22 Regulations.
At 12:40 p.m. the licensee began touring LPA's through the daycare room, LPA observed children toys, chairs and tables in the daycare room. LPA observed two safety gates were installed to the daycare room, one side leads to the kitchen and the other side leads to the hallway of bedrooms.REPORT CONTINUES ON NEXT PAGE 1 of 5 |
