25 | Licensing Program Analyst (LPA) Lilli Babcock conducted an unannounced Case Management inspection due to an incident that occurred on 8/24/22 and was reported to the Department on 8/25/22. Due to COVID- 19, precautionary measures were taken, and LPA wore appropriate personal protective equipment. LPA met with Director, Jennifer Campbell to whom the reason of the visit was explained and who guided LPA on a tour of the facility. Census was taken.
On August 24, 2022 an unusual incident occurred that involved an unauthorized male who entered facility grounds via an unlocked church door. The facility rents the space from the church and the facility is connected to the church via a set of double doors near the kitchen. The pastor accidentally left an exterior door to the church open, through which the individual entered looking for the pastor, and then entered the double doors to the facility. LPA observed both the church and facility are painted the same exterior color. Director ushered the individual out of the facility and children were indoors during the incident, although some of the children did hear the intruder and director yelling. Director reported incident to police and report was taken, however facility was not given any documentation other than a report number. Facility is working with the church regarding placing a lock on the double doors separating the facility and church. The church board is meeting on 9/14/22 to discuss the matter, and director will advise LPA on the outcome. In the meantime, the Director is checking the church doors before school starts and after school to help ensure no one is able to get into the facility unauthorized. The main entrance to the facility is via a metal security gate that requires staff admitting individuals. The facility reported this incident to the Department within the required 24 hours.
Based on information obtained during this investigation, the facility followed all proper procedures; Director promptly ushered the individual out of the facility, parents were notified, police were notified and a report made, and an incident report was submitted to licensing. LPA will follow-up regarding the church's decision regarding locking the double doors between the facility and church.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director, Jennifer Campbell.
