25 | Licensing Program Analyst (LPA) Lilli Babcock conducted an unannounced Follow-up Case Management inspection due to an incident that occurred at the facility on 6/17/24 and was reported to the Department on 6/17/24. LPA met with Director, Jennifer Campbell to whom the reason for the visit was explained. LPA toured the facility. Census was taken. There were 3 staff present caring for 23 children. The facility was observed to be operating within the license capacity limitations.
On June 17, 2024, an unusual incident report regarding personal rights was made to the Department. The facility reported this incident to the Department within the required 24 hours.
During the visit, LPA Babcock conducted interviews with 1 staff and 5 children and obtained documentation in the form of a children's roster and Personnel Report (LIC 500).
No deficiencies are being cited at this time.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
