Licensing Program Analyst (LPA) Elka Chavez conducted an unannounced annual inspection. LPA met with Site Supervisor, Fabiola Da Rosa who guided LPA on a tour of the facility. This is a preschool program licensed for 70 children. Per the Director there are 36 children enrolled. This program operates Monday - Friday from 6:30 a.m. to 5:00 p.m.

All areas identified on the facility sketch were inspected. Upon arrival, LPA observed 28 children with 6 staff. Teacher-child ratios were observed to be in accordance with Title 22 regulations. The facility is within the conditions, limitations, and capacity specified on the license. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times.

Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the site supervisor’s office is used as the designated ill isolation area. Parents are contacted immediately when children are determined to be ill. A first aid kit is kept in each classroom. Smoke detectors are present and fire extinguishers have been serviced in January 2023.

Menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. LPA observed they are posted in each classroom. And a copy is provided to parents a week before the beginning of each month. This program provides breakfast, lunch, and PM snacks. Meals are prepared on site. For drinking water, LPA observed a water jug and disposable cups in each classroom. LPA observed the food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage containers for solid waste, including moveable bins, have tight-fitting covers that are kept on, and are in good repair.

Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. LPA observed they in a cabinet with a child safety latch. Storage areas for poisons are locked. The facility has one or more functioning carbon monoxide detectors that meet requirements.

Outdoor activity space equipment and surface is maintained in a safe condition as is free of hazards. Areas around and/or under climbing equipment and slides have cushioning material to absorb a fall. LPA did not observe any bodies of water during this visit.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. LPA observed sign in and out is conducted in each classroom. Children’s records were reviewed for Emergency Card, Immunization Records, LIC 995 Notification of Parents’ Rights, LIC 701- Physician’s Report, LIC 613A- Personal Rights, and signed Admissions Agreement. All children's records were complete.

Facility records were reviewed for LIC 9040- Facility Roster, 9148- Earthquake Preparedness form, Daily schedule and Disaster drill log, last drill conducted on 07/23/2023.

Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC-501: Personnel Record, LIC 503- Health Screening, LIC 508- Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse, Transcripts or Permit and current Mandated Reporter Training Certificate. LPA observed that staff have proof of current Pediatric CPR and First aid certificates and Mandated Reporter Training (AB1207).

Site Supervisor, Fabiola Da Rosa was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. Site Supervisor, Fabiola Da Rosa was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on the LPA’s observations and records review, there are no deficiencies cited as a result of the Annual inspection on this date.
