On October 3, 2024 at 1:28pm, Licensing Program Analyst (LPA) Jonnisha Culbert conducted an unannounced inspection at the facility noted above and met with Licensee, Yvonne Gabriel. The purpose of the inspection was to conduct the random/annual inspection. The operating hours of the facility is Monday through Friday from 7:00 AM to 4:30 PM. Entrance Checklist (LIC 126) was provided to the Licensee upon arrival. Individuals residing in the home were discussed and noted. At the time of the inspection, 7 children, licensee, adult 1, and assistant 1 were present. All adults present during inspection were fingerprint cleared. At approximately 1:35pm adult 1 left.

All areas used by children was inspected. This facility is a single-family home that consists of three (3) bedrooms, 2 bathrooms, day care room, living room, dining room, kitchen, attached garage (converted to day care room), fenced backyard, and fenced area in backyard.

Areas that are accessible to children include: 1 bedroom, 1 bathroom, day care room, living room, dining room, kitchen, attached garage, and fenced backyard. Per Licensee, the children utilize the fenced backyard for outdoor activity.

Areas off-limits to children include: 2 bedrooms, 1 bathroom, and fenced area in backyard. LPA observed the bedroom doors closed and chained from top door, making the off-limit areas inaccessible to children in care. LPA did no observe bodies of water. Per licensee, there are no firearms on the premises. The Licensee was advised that off-limit areas must be made inaccessible during operating hours.

The following documents were posted in a prominent, publicly accessible area: Facility License, Notification of Parents' Rights (PUB 394), Emergency Disaster Plan (LIC 610A), and Earthquake Preparedness (LIC 9148). The Licensee has verification of disaster and fire drills available for review. The last drill was conducted on 10/03/2024. The Licensee also has a current Facility Roster (LIC 9040) available for review.

Areas used by children were inspected for safety, comfort, heating, cleanliness, and telephone service. LPA did not observe any wall heaters or fireplaces in the home. The home has central air and heating. Detergents, cleaning compounds and medicines were made inaccessible to children in care. Per Licensee, there are no poisons kept in the home. Licensee was advised that if any poisons are purchased, it is required to be locked with a key or combination lock. Per Licensee, there are no pets, firearms and/or weapons on the premises.

LPA observed age-appropriate toys and napping equipment for children. LPA observed electrical outlet covers installed in the childcare areas. LPA observed two required fire extinguishers (2A-10BC) with no purchase date or service tag. Per licensee, they had the receipt laminated and attached to the fire extinguisher, but they do not know what happened to it. LPA advised licensee to purchase a new extinguisher or service current one to show proof of purchase date or service date for future inspections. Licensee was reminded to have the fire extinguisher serviced yearly. Smoke detector and carbon monoxide detector was not tested due to children napping. Licensee stated that she provides meals and snacks to the children. Licensee was reminded that food that is brought from the child's home shall be labeled with the child’s name and properly stored or refrigerated.

LPA conducted a record review of 6 children's records, Licensee, and assistant’s record. Based on the record reviews, all children records were complete. Licensee and assistant has proof of immunization against measles and pertussis. LPA did not observe TB clearances for licensee or assistant. Per licensee, she knows that they had them but could locate them. LPA reviewed previous inspection and noticed that licensee’s TB clearance was noted on inspection. LPA advise licensee to keep TB clearance inside of personnel files. Licensee and assistant has a declination on record for the influenza. They have a valid Pediatric First Aid and CPR certification (licensees expires 03/29/2025 and assistance expires 03/29/2025). Licensee completed the required Preventative Health and Safety Practices on June 26, 2016 and Mandated Reporter Training (AB 1207) on 01/29/2024. Assistant complete mandated reporter training on 12/11/2023Licensee was advised that the Mandated Reporter training (AB 1207) must be completed every 2 years and is available at www.mandatedreporterca.com.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/.Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

No deficiencies were cited today. During the exit interview, Yvonne Gabriel, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. A notice of site visit was given and must remain posted for 30 days.
