25 | Licensing Program Analyst (LPA) Stephanie Li conducted a case management inspection for physical plant due to a construction incident that was reported to licensing on 12/17/24 and incident construction was to occur on 12/23/24 through 1/6/25. LPA met with Educational Specialist Denise Manaloto. She was the previous director and filling in for current Director Cassandra who was effected by the Eaton fire. Denise gave a tour of the facility. Census was taken. There were 26 children and 4 staff during the time of visit. Risk assessment was done.
The construction that was reported stated that a wall in Room 3 will be taken down so children can have direct access into the restroom, eliminating the need for Room 3 children to walk through Room 1 classroom to get to the restroom and putting staff out of ratio. Additionally, an outdoor gate will be replaced and an outdoor sink will be installed.
LPA conducted interview with staff and took photos of the newly de-constructed wall. LPA observed that part of the wall in Room 3 was de-constructed to fit a door way so Room 3 children have direct access to the shared bathroom. Room 1 and Room 3 are connected with the bathroom in the middle as a partial divider. Half doors were installed in the door ways to allow children privacy while allowing teachers to maintain supervision. LPA did observe a metal frame protruding from the floor where the carpet ends and advised to be mindful of children walking out of the restrooms with out shoes could injure their feet. Staff stated they will cover the metal with duct tape. Staff stated that they are also planning on replacing some of the carpet in front of the new restroom doorway. LPA informed Denise that the new entryway into the restroom from Room 3 will need to be identified on an updated facility sketch, LIC999. Staff indicated that the outdoor sink and replacement of the outdoor gate has not happened yet.
The facility is in compliance of Title 22 regulations. Incident report was sent in properly and timely. No citations are being cited during his visit.
Exit interview was conducted with Denise Manaloto. |
