25 | REQUIRED- 1 YEAR INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced required 1- year inspection to the above facility on 09/01/23. LPA arrived at the facility at 11:30 AM and met with Liana Shahnazaryan, Licensee who guided analyst on a tour of the facility. A COVID 19 risk assessment was conducted prior to entering the facility. Also present during this inspection, were Meline Sargsyan, Licensee’s Assistant and Sevada Antonyan, Licensee's husband. Per licensee, facility hours are Monday to Friday from 7:00 a.m. to 6:00 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee.
This is a one story home located on the first level. The home consists of 3 bedrooms, 2 & 1/2 restrooms, living room, dining room, family room, kitchen, laundry room, backyard (fenced), front yard, storage in the backyard, and detached converted garage in the backyard. The children use the bathroom in the hallway, living room, dining room, family room, 2 bedrooms, backyard (fenced). Per licensee, areas off limits to children and parents include: 1 bathroom, 1 & 1/2 bedrooms, kitchen, front yard, storage in the backyard, laundry room, and detached converted guest house. LPA did not observe any stairs in the home. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 11:50 a.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room and dining room, LPA observed sofas, a coffee table, a TV, a dining table and chairs in the living room and dining room. There is a fireplace in the living room which has been blocked off to prevent access to children. Next LPA toured the family room and observed a sofa, children tables and chairs in the family room. LPA observed a child safety gate was installed to the kitchen to make the off-limit kitchen, laundry room and half bathroom inaccessible to the children. Next LPA toured the two daycare bedrooms in the hallway and observed children chairs, a piano and toys in the bedrooms. Next LPA toured the daycare bathroom in the hallway of bedrooms and observed child proof safety latches on the cabinets under the sink.
