Licensing Program Analyst (LPA) Roxana Lopez conducted a case management inspection. LPA met with Maria Flores, Licensee who guided analyst on a tour of the facility- census was taken. The purpose of this inspection was to ensure that licensee received a copy of Decision and Order for Johana Flores dated January 30th 2025, with an effective date of February 10th, 2025..

LPA informed the Licensee that the purpose of this inspection is to inform her that the Decision and Order excludes Johana Flores from presence in the Child Care Facility. Johana Flores is prohibited from employment in, presence in, and contact with clients of, any facility licensed by the Department of Social Services, or certified by a licensed foster family agency, and from holding the position of member of the board of directors, executive director, or officer of the licensee of any facility licensed by the Department. The exclusion is for 2 years effective date of February 10th, 2025.

Per Licensee, they received a copy of the Decision and Order, but since it was address to her daughter she sent pictures of it to her. LPA explained the Decision and Order to licensee- Per Licensee, they understood and requested to review her roster. Licensee, requested for two people (including Johana Flores) to be separated from her license.

At this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Licensee Maria Flores .------- pg. 1 of 1 ------------------
