This was a complaint inspection conducted by Denise Gibbs, Licensing Program Analyst (LPA) on 5/21/2021 at 3:00 PM. Due to COVID-19 and precautionary measures, this complaint was conducted with Tiffany Fields, Director via Zoom for the purpose of delivering findings.

During the course of this investigation, LPA interviewed Director, staff, parents, and children. All pertinent documentation was collected. No disclosures were made by interviewed parties. LPA observed that the facility is following COVID-19 precautions and procedures. Parents were informed about COVID-19 procedures in writing, throughout the addition to the parent handbook and through a post in the Bright Wheel application used daily to communicate with parents. Through interviews and observation it was confirmed that facilities does have a plan for social distancing among the children. Groups are split up and kept small, play areas are designated for a maximum number of children at a time and children have individual activity containers. Social distancing is encouraged with visual cues such as tape on tables and pictures of children.

Interviewed parties are aware that the front gate remains locked throughout the day for safety precautions and if entrance is needed, there is a door bell that is used and staff opens the gate as soon as they can. During the tour of the facility

LPA observed that the facility was clean and free of clutter. Facility has cleaning supplies and sanitizer. Per staff and staff schedule, the facility is cleaned and sanitized by the opening teacher, again when the next teacher arrives, by the closing teacher and throughout the day as needed. Diapers in the younger preschool classroom are disposed of, first in a bag then in a diaper genie. Staff then throws soiled diapers outside in the trash bin throughout the day. Per staff, changing table and toilets are sanitized after each use.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove
the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.

No deficiencies will be cited today 5/21/2021.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Director, Tiffany Fields, via Zoom, during which appeal rights were explained. This report along with a copy of the appeal rights will be sent to the Director via email with a read receipt or confirmation of email, which will act as the Director's signature. -----------PAGE 2
