13 | Licensing Program Analyst (LPA) Randy Derraco conducted an unannounced complaint inspection to concluded the investigation and deliver findings to director Tiffany Fields on 09/15/21 at 2:55 PM. LPA observed 23 children in care and 4 teachers (5 adults total). The child care center was observed to be clean and in good repair.
Based on the results of the investigation, LPA was unable to confirm personal rights of a child was violated and the physcial plant of the center was compromised. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Tiffany Fields.
