Annual Inspection was conducted in Armenian.

On 12/30/24 , at 1:50 pm Licensing Program Analyst (LPA) Shushanik Safaryan conducted an annual random inspection to the above facility. LPA met with Licensee, Satik Alikhanyan , to whom the reason for the visit was explained. Per Licensee, there are 14 children currently enrolled. There were 10 children and two adult (licensees’ assistants) present upon arrival. The licensee was observed to be operating within the license capacity limitations.

The licensee states that 4 adults and no children currently live in the home. All adults living in the home have obtained a criminal record clearance. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
This is a two-story home. The second level home consists of 2 bedrooms, restroom, living room, dining room, kitchen, and patio, first level of the home consists of day care room , laundry room, 1 bedroom, 1 bathroom, kitchen and converted garage which consist of 1 room and bathroom, front yard, and backyard (fenced).

Children use the first level of the home which consist of 1 day care room , bedroom ,bathroom in the bedroom, laundry room and fenced backyard. Parents enter the home through the rear of the home through the driveway which leads to the backyard and day care room. LPA observed door connecting to the stairs that leads to the second level had a child safety latch . LPA observed child safety latch on the laundry room door and child safety gate blocking kitchen access to the children .

During this visit, all areas identified that are accessible for children to use were inspected for the safety, comfort, and cleanliness. At 2:00 pm, Licensee guided LPA to the tour of the home.

LPA was toured starting from side gate where parents enter . LPA toured day care room , where observed children’s toys, small table ,chairs. Next LPA toured the bedroom , where observed four play yards and cots for napping . LPA observed 2 children sleeping in play yards . Next LPA toured the bathroom available for the children and did not observe any hazards. LPA observed child safety gate separating kitchen . In the kitchen , LPA observed licensee`s assistant preparing food. Next LPA toured the laundry room and did not observe any hazards.
Facility License, Emergency Disaster Plan, Parent’s Rights Poster, and Earthquake Preparedness Checklist were observed to be posted in the day care room on the wall . A current children’s roster was available for review.
There is telephone service via a cellphone that stays at the facility during operation hours. Hours of operation are Monday – Friday, 7:00 am – 11:00 pm. There is central air and heating in the home.
Detergents, cleaning compounds, and medications were observed to be inaccessible to children. Per Licensee, there are no poisons in the home. LPA advised that any poisons should be locked under key or combination lock. Licensee states that there are no firearms or weapons stored in the home.

The valve on the required 2A 10BC fire extinguisher indicates fully charged and serviced with the service date 08/02/2024. Smoke and carbon monoxide detectors were tested and are operable.

All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Last drill documented was conducted on 12/18/2024.

The home is observed to be clean and orderly. There are first aid supplies available in the day care room. There are age-appropriate toys available for children.

Per licensee, she provides food for children in care. Licensee was advised that if food is brought from the children’s homes, all containers must be labeled with child’s name and properly stored or refrigerated.
Currently, children are using the backyard for outdoor play time.

There is a swimming pool on the side of the house. The back yard is fully fenced with a swimming pool that is enclosed with a five-foot rod iron fence with a gate that opens away from the swimming pool and self closes and self-latches.

LPA observed locks on the gate doors that leads to the swimming pool. LPA observed gate on the end of the property that leads to the swimming pool gate and to the stairs to the second level. LPA observed gate was locked with the lock . Per licensee , she keeps lock closed and key inaccessible to the children . LPA observed sliding door that leads to the backyard have a door alarm. Licensee stated full supervision provided all the time .

During the visit , LPA provided information regarding AB 2866 for new Pool Safety requirements to licensee . LPA informed the licensee that follow up visit will be conducted to make sure licensee complies with the requirements indicated in AB2866.
The outdoor play area was observed to be fenced and gated. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA did not observe any objects that can pose danger to children.
In the yard LPA observed detached garage , per licensee it is not used for the day care. All adults leaving in the detached garage have obtained criminal record clearance .
At 2:26 PM, Licensee and staff records were reviewed. The licensee has completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 08/2026.

At 02:25 pm, LPA observed that the Licensee does have proof of Mandated Reporter AB 1207 compliant Child Care Training Certificate on file with expiration on 08/31/2024. LPA reminded licensee that certificate expired and needs to be renewed every 2 years.www.mandatedreporterca.com


At 2:30 PM, children`s records were reviewed, including emergency information, and were observed to be complete. Licensee states she is currently not caring for any infants. LPA discussed with the licensee if she does not renew her liability insurance , parents need to be notified and provided with LIC 282.

LPA created an LIC 857, Children’s Record Review, and LIC 859, Staff Record Review which documents staff and children’s files reviewed during this inspection.

Per licensee, there are no animals on the premises.
Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home.

LPA did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that fall into these categories are not permitted in a family childcare facility.

Per licensee , she does not have any children that needs Incidental Medical Services currently.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA advised the licensee how to access forms, regulations and quarterly updates on the Child Care Licensing website at www.ccld.ca.gov.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California

During the exit interview, the LICENSEE Satik Alikhanyan, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.


Based on the LPA’s observations and records review, the following deficiency listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

LPA discussed all the deficiencies cited during today’s inspection with the licensee. Per licensee, she understands all the deficiencies and plan of correction .

A notice of site visit was given and must remain posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
