13 | On July 22, 2025, at 11:00 AM, Licensing Program Analyst (LPA) Dayna Chambers conducted an Unannounced Complaint Inspection for the purpose of delivering findings for the above allegation. LPA announced the purpose of inspection and was allowed entry to facility by Jonathan Torres, Assistant., who guided analyst on a tour of the facility. Also present was the assistant Cinthia Mellisa Valladares. There were 5 children present with 2 staff upon arrival. The licensee was unable today due to family emergency. During the investigation LPA obtained a copy of LIC9040 children roster, reviewed children's files, toured and observed the facility, and conducted interviews with staff, children, and parents.
The information provided by the reporting party alleges that facility staff hit day care child with an object.
Based on the LPA's observations, record review, and interviews conducted, it was found there is no evidence to confirm the allegation. LPA conducted interviews and no information to corroborate the allegation; therefore, allegations could not be substantiated. LPA observed children in care on 07/22/2025 and 05/19/2025, children appear to be happy while in care. Based on observations, the LPA did not observe any violation of personal rights.
