On 04/13/2023 at 9:30 AM, Licensing Program Analyst (LPA) Lilli Babcock conducted an unannounced required inspection. A COVID risk assessment was conducted, and appropriate Personal Protective Equipment (PPE) was used by LPA. LPA met with Director, Maribeth Peralta, who guided LPA on a tour of the facility. This is a preschool age program. Facility is currently licensed for a capacity of 124 children and operates Monday – Friday from 6:30 AM – 6:30 PM. Per the Director there are 121 children enrolled.

All areas identified on the Facility Sketch were inspected. The following staff were present during this inspection: Room 2 (2-year-old room): Staff #1, Staff #2, Staff #3, and Staff #4 with 34 children; Room 3 (3-year-old room): Staff #5, Staff #6 and Staff #7 with 34 children; Room 4 (4-year-old room): Staff #8, Staff #9, Staff #10, and Staff #11 with 35 children. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license.

LPA observed the facility to be clean, sanitary and in good repair. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. Napping equipment (cots) and bedding

were inspected for good condition, appropriate storage, and cleanliness. At 10:02 am LPA observed the children’s cots in Room 2 to be stacked on top of one another, and in Room 3 the children’s cots were observed to be stacked on their sides. In both Room 2 and Room 3 the children’s bedding was on the cots. In Rooms 2 and 3 children’s bedding were observed to be touching other children’s bedding and the bottom of the cots were observed to be touching the top of the children’s bedding. Children take their bedding home to wash twice weekly or as needed. Storage for children's belongings were inspected. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, a sick room next to the office is used as an isolation area. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

Food menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides breakfast, lunch, and 3 snacks during the day. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and LPA inspected that any contaminated food is discarded immediately. At 10:27 am, LPA observed unlabeled food in baggies of pancakes, bagels and English muffins in the refrigerator and unlabeled baggies of crackers, cereal, cheerios, and macaroni in the dry-storage pantry. No expiration date for the contents of the baggies was observed by LPA.

Children in all 3 classrooms bring refillable water bottles with names on them from home and there is water in a jug available to refill the water bottles as needed. Children’s water bottles from the classroom are taken outside during outdoor play time, and the water jug is taken outside to refill the bottles as needed. All storage containers for solid waste, including moveable bins, have tight fitting

covers on and are in good repair. Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in an area inaccessible to children. Director states that there are no poisons stored at the facility.

At 9:25 am LPA observed a smoke and carbon monoxide detector were tested and were operable on this day.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There is adequate shade in the play yard.


Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. At 10:41 am, LPA observed Child #1 (C1) was signed in on 4/4/23 but was not signed out. LPA also observed parents signing their children in and out using initials and not a full signature.

Staff Records were reviewed to ensure that a health screening report and immunizations are on file. At 2:15 pm LPA observed that Staff #3 (S3), Staff #5 (S5), and Staff #6 (S6) did not have proof of the required core classes needed to qualify as a teacher. Verification of CPR/First Aid and Mandated Reporter training documentation was reviewed.

Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA advised the facility representative to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.



Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on the LPA’s observations and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code od Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
