25 | Licensing Program Analyst (LPA) Lilli Babcock conducted a Case Management-Deficiencies visit on 1/9/25 to address deficiencies revealed during a complaint investigation. A COVID risk assessment was conducted. LPA met with Director, Maribeth Peralta. Census was taken. There were sixty-eight (68) children and eleven (11) staff present at the facility during this inspection.
Interviews conducted during a complaint investigation revealed that on 11/19/24, a child (Child #1/C1) from the preschool program was hit in the forehead by a metal water bottle thrown by a peer. The water bottle hit the forehead of C1 causing a cut that required stitches. LPA asked Director for all of the facillity incident reports for C1. LPA was provided an incident report dated 11/19/24 for the incident where "During outside playtime, C1 was playing with some friends. A child decided to throw a water bottle in the air which resulted in the bottle hitting C1 on the forehead causing a deep cut". As of this date, the incident involving C1 was not reported to licensing as required. During interviews, Director, Maribeth Peralta confirmed the incident did occur and the incident was not reported to licensing.
Based on interviews and records reviewed, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.
A notice of site visit was given and must remain posted for 30 days.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
