25 | Licensing Program Analyst (LPA) Lilli Babcock conducted an unannounced Case Management inspection due to an incident that occurred on 10/18/24 and was reported to the Department on 10/21/24. A COVID risk assessment was conducted. LPA met with Director, Maribeth Peralta to whom the reason for the visit was explained. Director guided LPA on a tour of the facility. Census was taken. There were 9 staff present caring for 68 children. The facility was observed to be operating within the license capacity limitations.
On 10/21/24, an unusual incident report was made to the Department regarding an incident on 10/18/24 that involved a child who ran out of the classroom. The facility reported this incident to the Department within the required time frame. Based on information obtained during interviews conducted with staff on 1/9/25 and 1/16/25, LPA Babcock determined that Child #1 (C1) ran out of classroom #4 and up the stairs to the landing which leads to the office door or the door/gate to the parking lot. Staff #1 (S1) observed C1 run out of classroom #4 and ran after C1. C1 opened the door/gate and ran toward the parking lot. A second gate was open as it was being worked on by a maintenance crew. C1 ran past the second gate, followed by S1 and S1 was able to catch up with C1 approximately 5 feet from the walkway past the second gate. S1 immediately brought C1 back to the classroom and parent of C1 was notified. According to interviews S1 was able to maintain visual contact of C1 at all times. During the visit, LPA obtained a copy of the incident report given to the parent of C1 and the written report (LIC 624) for licensing.
No deficiencies are being cited at this time.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
