This is an infant program licensed for 19 children including a preschool and school-age component; this program operates Monday through Friday from 6:00 AM to 6:00 PM. The Assistant states there are 9 infants currently enrolled. The facility is located behind a shopping center and is not attached to any other businesses. LPA observed a total census of 9 children with 2 staff members.

The following classroom inspected was the infant. All staff members are fingerprint cleared. Licensed facility is within the conditions, limitations, and capacity specified on the license.

The classroom furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. Indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. Telephone service, heating, lighting, and ventilation were evaluated.

There is storage for children’s belongings. Infants have enough cribs with firm mattresses in their own designated area, as well as children’s cots for infants over a year old. Fitted sheets are supplied by the facility and laundered weekly and as needed.

LPAs observed the 15-minute sleep documentation only for infants 12 months and under. At 2:30 pm LPAs observed sleep logs for 2 of the 8 infants present during this visit. page 1

• Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.

• Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

LPA was informed that there are currently no children on medication at this time, however the Director has an IMS plan and LPA reviewed and provided consultation. Director stated that medication would be kept in a medication lockbox in her office and staff is aware where it is kept and how to administer. LPA advised that Medication should be given back to parents/guardians when child leaves the facility or medication expires.

Outdoor playground equipment is in safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There are currently 2 playgrounds, and 1 playground is designated for the infant component. The infant playground has an infant equipment with imitation grass turf to cushion for falls. There is adequate shade in the play yard with awning and shade over the play equipment. There is enough play equipment and toys for the children to use in the play yard.

Sign in/out is done electronically using a designated parent code for each family and a back-up sign-in/out sheet is available should the electronic pad is not functioning. A printout is available for review.

LPA reviewed staff records and found that they were complete. Clearance for adults and verification of CPR/First Aid, educational requirements, immunization records and health preventative practices documentation was reviewed. Children’s records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. LPA observed Infant Sleep Plan for children who were enrolled prior to their first birthday. There is also a Needs and Service Plan conducted when entering the program, every 3 months and as needed. LPA advised Director that selected staff and not all staff require the 8-hour Health, Safety, Nutrition and Lead training.

Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018, any person who works in a childcare facility shall complete the training and renew the training every 2 years. Website provided:

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Staff states they document infant sleep for infants up to 12 months old, as printed on their current infant sleep log sheets. LPAs discussed infant sleep log documentation.

Availability of drinking water was reviewed and per staff member parents provide water from home and refills are available by the use of bottled water.

There is a changing table with arm’s length sink for hand washing. LPAs observed the paint on the wood border of the infant’s changing table to be chipped with bare wood exposed in numerous places. LPAs advised to have infant changing table refinished or repaired to ensure that infants do not get splinters from rubbing against the bare wood, while having their diaper changed.

Diapers and wipes are supplied by parents. Toys and surfaces are observed to be safe and clean. Staff restroom is available near the main lobby of the entrance.

Combination carbon monoxide and smoke detectors were present in each classroom throughout the facility. LPAs observed while Director tested and were found to be operable during the time of the visit. Fire extinguishers are serviced annually.

Storage cabinets in classrooms were inspected, and no hazardous items were found, cleaning items are kept in the cabinets which are locked with child proof locks and inaccessible to children in care. No poisonous cleaning compounds were found in the childcare center. Emergency disaster drill was last conducted on June 4, 2025. There is a First Aid kit available in the kitchen and the school office; and supplies were inventoried and found to be in good condition.

There is a kitchen area where meals are prepared and distributed to each classroom. Food preparation areas were toured for safety, cleanliness, and proper equipment. Breakfast, Lunch and PM snacks are served by the facility.

Based on the LPAs observations and record reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Betty Jimenez.
