Licensing Program Analyst (LPA) Anomeh Eivazian contacted an unannounced complaint inspection to the above facility for the purpose of delivering the complaint finding. On 04/11/2022 LPA arrived at 10:18 a.m. met with Crystal Castaneda, director who guided analyst on a tour of the facility. During this inspection there were 14 children present in the facility with 2 staff.

During this investigation, LPA Eivazian conducted interviews with staff. Also LPA obtained a copy of facility roster.

Based on staff#1 and staff#2 interviews, during the week of 03/07/2022, staff#2’s biological child was present at the facility who is not enrolled in the preschool or infant program. Per staff#1 and staff#2, staff#2’s child never came around the daycare children and stayed in the staff room and did homework.

Based on LPA observations and interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated at this time. A type B deficiency was issued on this date based on California Code of Regulation, 101161(a)—Limitations on Capacity and Ambulatory Status. Deficiency that is being cited need to be cleared to protect the children’s health & safety.

The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the Crystal Castaneda, director at 11:00 a.m..
