On 04/25/2024 , Licensing Program Analyst(LPA) Shushanik Safaryan conducted an unannounced visit regarding complaint investigation received by the Department .During the visit , LPA met with Facility Representative , Aracely Ramirez and Lisa Marie Scott to whom the purpose of the visit was explained. Facility Representative guided LPA on the tour of the facility , LPA observed 58 children with 13 staff members.
Complaint Alleged that the facility is in disrepair . During the visit, LPA interviewed Director assistant and
5 staff members , obtained children roster , notification to the parents for the maintenance issue . Based on the information obtained and interviews conducted facility was in disrepair on 04/22/2024 starting from the morning . Facility toilets were not operable and children and staff had to walk in groups to another licensed facility located at 72 W Bellevue Drive until the issue was fixed .

Per Facility Representative and information obtained issue with the restrooms was fixed same day at 04:20 pm .

The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.
Based on interviews and information obtained , the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 , Chapter 1, Article 7 , section 101238 (a) Buildings and Grounds are being cited on the attached LIC 9099D.

Exit interview conducted with Facility Representative ,Aracely Ramirez and Lisa Mari Scott and Appeal Rights explained.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.

Per Facility Representative issue was fixed same day by the facility maintenance person same day . Proof obtained .

Based on the interviews and information obtained Facility did not have operable toilets for the staff and children . Children had to walk to the different facility located in the different address to use the restrooms .
