Licensing Program Analyst (LPA) Alanna Gontarek conducted an unannounced annual inspection to the above facility. LPA met with Susann Beck, Licensee who guided analyst on a tour of the facility indoors and outdoors. Also present during this inspection, was Joy Jasper, Licensee’s Assistant, Licensee's husband, and two sons under 10 years old. Per Licensee, there are 15 children that are currently enrolled. The Licensee is within the conditions, limitations, and capacity specified on the license. A current children’s roster was available for review. There were 8 children present upon arrival. Hours of operation are: Monday to Friday from 07:00 a.m. to 05:30 p.m. LPA observed staff wearing face masks. LPA advised to have children 2 years and older wear a face mask and keep a distance of 6 feet social distance due to COVID-19 preventative measures.

This is a one-story home which consists of 3 bedrooms, 2 bathrooms, kitchen, dining room, living room #1/ daycare area, living room #2, front yard, back yard (fenced), and two side yards in backyard. Daycare areas include: the bathroom in the hallway, living room #1, living room #2, kitchen, room #1, room #2, outdoor small shed , one side yard, and back yard. LPA observed that there are two fireplaces; one in living room #1, and one in living room #2. Both fireplaces are barricaded with a metal fence secured to wall. Per Licensee, areas off limits to children and parents include: Master bedroom, master bathroom, garage, and large outdoor storage. The licensee provides fresh perishable and non-perishable food in the home in good quantity and quality to meet the needs of the next 3 meals and between meal snacks.

The licensee states that one additional adult and 4 children currently live in the home. Per Licensee, she currently has one assistant. All adults present in the home have obtained a criminal record clearance or exemption prior to working, residing or volunteering in the licensed Child Care home. Licensee states there is one firearm in the master room, ammunition stored separately and locked. Licensee states there are 2 firearms in the off-limit garage stored in an executive safe, ammunition stored separately and locked. LPA.

observed lock box requiring fingerprint to unlock. Home maintains appropriate fire clearance.

All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. There is telephone service via cellphone that is used and the cellphone stays at the facility during operation hours. There is ventilation and heating (central). LPA observed two feeding chairs and two pack-n-plays in living room #1. LPA observed a first aid kit and additional face masks in living room#1. LPA observed one Pack-N-Play in Bedroom #1 and one Pack-N-Play in Bedroom #2.

LPA observed detergents, cleaning compounds, and other items which could pose a danger to children to be inaccessible to children. LPA observed medications stored in the bathroom accessible to children. LPA advised to keep medications stored in inaccessible master bathroom. LPA observed cleaning compounds stored under the kitchen sink, made inaccessible by a child safety latch. LPA observed poisons stored in the garage, made inaccessible with a key lock on garage door. LPA advised that storage areas for poisons must be locked with a key or combination lock at all times. The restroom that children use was observed to be safe and sanitary.


Fireplaces and open face heaters are inaccessible to children. LPA observed two fireplaces to be barricaded with a metal fence, and secured to the wall. The valve on the required 2A 10BC fire extinguisher indicates fully charged and was purchased on 12/31/2020 . Smoke and carbon monoxide detectors were tested and are operable.

The home is observed to be clean and orderly. There are age appropriate toys available for children in daycare room/living room #1 and bedrooms #1 and #2. Children have individual mats, Pack-N-Plays, and one play yard for outdoor area. Licensee states there are no children currently in care that nap, but licensee provides: mats, clean linens, blankets, and pillows that are in good repair, and LPA observed sleeping materials in the outdoor small accessible shed.

Currently, children are using the back yard and side yard for outdoor play time. The outdoor play area was observed to be fenced and shade available. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA did not observe any objects that can pose a danger to children on the outdoor yard. The licensee states that supervision is always provided. LPA did not observe bodies of water,

or pool. LPA observed one mini pool and 2 water tables stored in outdoor small shed. LPA advised Licensee to dump water out when children are done playing during water play. The licensee and other personnel have completed training on preventive health practices including Pediatric CPR and First Aid. The licensee's Pediatric CPR and First Aid expires on 3/2023. The licensee's assistant CPR/ First Aid expires on 2/2023.

Children’s records were reviewed, including emergency information and were observed to be complete.


The licensee, and licensee's assistant, do have proof of immunization against influenza, pertussis, and measles. LPA observed the Mandated Reporter AB 1207 compliant Child Care Training Certificate on file. LPA issued a Confidential Names List (LIC 811) to the licensee which documents staff and children’s files reviewed during this inspection.

All homes shall conduct fire and disaster drills at least once every six months, and document the date and time of each drill. Last drill documented was conducted on 5/27/2021.

There are 3 large dogs on the premises. LPA observed dogs stay in a locked side yard, inaccessible to children in care, during operating hours.

Emergency Disaster Plan, Parent’s Rights Poster and the Facility License were observed to be posted.

LPA did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, Trampolines and/or any other item that fall into these categories are not permitted in a family child care facility.

Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home.

LPA observed and inspected sleeping equipment for infants. LPA observed 6 Pack-N-Plays, available for each infant who is unable to climb out the play yard. All equipment meets the US Consumer Product

Safety Commission safety standards. LPA observed Pack-N-Plays/ play yards are free from loose articles and objects. There are no objects hanging above or attached to the side of the Pack-N-Play. LPA observed that the play yards do not hinder the entrance or exit to and from the space they are sleeping in. Mattresses were observed to be firm and covered with a fitted sheet that is appropriate to the mattress size. Each infant has their individual bedding and is washed weekly as required. Soiled bedding is replaced when wet or soiled and is placed in an area inaccessible to infants. Pacifiers were observed to not be attached to anything. LPA advised Licensee to label all pacifiers when a child has one. Licensee was advised that infants shall not be swaddled while in care and all infants up to 12 months should be placed on their back for sleeping. LPA advised the licensee to sleep infants where they can always be directly supervised and the door should remain open at all times. Licensee states that infants sleep in Bedroom #2.

Licensee states that she is currently caring for infants. LIC 9227 (Individual Sleeping Plan) for infants up to 12 months was explained and issued to the Licensee. Title 22 Regulation Section 102425(j) Infant Safe Sleep was discussed with the Licensee, including but not limited to documentation that shall be maintained. LPA provided the licensee with a copy of the Child Care Provider’s Guide to Safe Sleep, by American Academy of Pediatrics and Helping you to reduce the risk of SIDS. LPA also consulted and explained Child Abuse Reporting, Never Shake a Baby, and Safe Sleeping practices.



Incidental Medical Services (IMS):
The licensee states that she does not provide IMS. Per licensee, there are no children enrolled that require IMS at this time. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Based on the LPA’s observations, interviews, and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

Exit interview was conducted with Susann Beck, Licensee, including, but not limited to Appeal Procedures and Rights, Notice of Site Visit, and all Licensing reports (LIC 809s). The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
