25 | At 2:30 pm Licensing Program Analyst (LPAs) Lilli Babcock and Jessica Hopkins-Hernandez conducted an unannounced required 1-year inspection to the above facility. A COVID risk assessment was conducted. LPAs met with Licensee, Farkhondeh Goudarzi, to whom the reason for the visit was explained. Per Licensee, there are 12 children currently enrolled. Also present during this inspection, were Licensee’s two assistants and 11 children. The licensee was observed to be operating within the license capacity limitations.
The licensee states that 2 adults and no children currently live in the home. Per Licensee, there are two assistants in the facility. All adults living in the home have obtained a criminal record clearance.
This is a two-story home which consists of 3 bedrooms located upstairs, 3 bathrooms, kitchen, two outdoor sheds, detached garage, dining room, living room, playroom, front yard, side yards, and backyard (fenced). The daycare areas include: one bathroom near the kitchen downstairs, living room, one bedroom upstairs (used as a sleeping room for infants, playroom, kitchen, dining room, and backyard/sideyard. Per Licensee, areas off limits to children and parents include: 2 Bedrooms, 2 bathrooms located upstairs, two outdoor sheds, and detached garage.
Licensee’s daughter/assistant guided analysts on a tour of the facility at 2:50 pm. During this visit, all areas identified on the facility sketch that are accessible for children to use were toured and inspected and the following was observed.
