staff or children wearing face masks. LPA advised to wear face masks daily for preventative measures against COVID-19 virus. Based on LPA’s observations, from inspection on 2/18/21, the left side gate, leading into an off-limit area, was open at 12:50 p.m. LPA observed an open container of car fluid at 12:32 p.m. and garage door was open, containing tools, at 12:50 p.m. The side area was accessible to children in care. Pictures were taken. This poses a potential health and safety risk to the children in care.
On this date, 2/22/2021, LPA observed all children and staff wearing face masks. LPA also conducted an interview with Licensee, obtained licensee declaration and obtained facility roster via email.
Based on LPA’s observations and interviews conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 & Section 1 ), are being cited on the attached LIC 9099D.
Exit interview was conducted with Licensee Cynthia Flores at 4:05 p.m. A copy of this report (LIC 9099), Appeal Rights and Notice of Site Visit were scanned via email to the Licensee, who understands that an electronic “Read Receipt” and/or confirmation of receipt of the e-mail confirms receipt of the report constitutes as an electronic signature.
A "Notice of Site Visit" and copy of the report was issued via email. Notice of Site Visit must remain posted for 30 days. Failure to do so will result in a $100.00 civil penalty. This report must remain on file for three (3) years. This report must also be posted for 30 days in clear view, at the entrance of the Family Child Care Home. The report cannot be covered/concealed. |
