25 | On 9/23/25, at 9:30AM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an Unannounced Annual/Random Year inspection to the above facility. Upon arrival, LPA disclosed the purpose of the inspection and met with Licensee, Mariam Armenakyan, who guided the LPA on a tour of the facility. Adults in the home were discussed and all have criminal record clearance. Also present was, Licensee’s Assistant / Aldona Avagyan. There were 6 day care children present during today’s inspection. Licensee states that there are currently 14 children enrolled. The children's roster was reviewed and is current. Per licensee, the facility’s hours of operation are Monday - Friday, 8:30AM - 5:30PM. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Disaster drill log was also available during today’s inspection, last disaster drill conducted on 9/6/25.
This is a single story home which consists of 3 bedrooms, 2 bathrooms, living room, dining room, kitchen, front yard, 2 side yards and a back yard. The home does not have a garage or a laundry room. According to the Licensee, parents drop off and pick up their children at the main entrance, which leads to the living room. Areas that are accessible to children are as follows: Bedrooms 1 - 3, bathroom 2 (which is located inside of bedroom 3), living room, dining room, kitchen, 2 side yards and back yard. There is a decorative fireplace located in the dining room which has been disconnected/removed. The side yard (on the right side of the home facing the street) has artificial grass with age appropriate toys. Per Licensee, off-limits areas are as follows: Bathroom 1 (located inside of bedroom 1) and front yard. The off-limits bathroom is made inaccessible by a child proof door knob/lock.
All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. There is a working telephone maintained in the home. LPA observed age appropriate toys, free of loose and sharp parts. LPA observed electrical cover outlets all through the home. Detergents, cleaning compounds, medications, and other items
