25 | CASE MANAGEMENT INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management inspection to the above facility on 02/14/2022 due to licensee is requesting capacity increase. LPA arrived at the facility at 11:40 AM and met with Ani Akopian, Licensee who guided analyst on a tour of the facility. Per licensee, facility hours are Monday to Saturday from 7:00 a.m. to 10:00 p.m.. Fire Clearance for capacity of 14 was granted on 02/03/2022.
This is a one story home consists of 3 bedrooms, 2 restrooms, living room and kitchen in the main home. Also, there is a detached guest house in the backyard which consists of Great Room (daycare room), 1 bedroom, kitchen in in the Great Room, 1 bathroom, two car garage in the backyard, front yard, and backyard (fenced). Per licensee the children use entire detached guest house which consist of 1 bathroom, Great Room (day care room), 1 bedroom (day care room), kitchen in Great Room, and backyard (fenced). Per licensee, parents enter the facility through the side gate which leads to the backyard and detached guest room (day care area). Per licensee areas off limits to children and parents include: Entire main front home which consists of three bedrooms, living room, two bathrooms, kitchen, front yard, and two car garage in the back yard. **Rooms that are off-limits need to be made inaccessible during operating hours**. LPA did not observe stairs in the home during this inspection. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 11:50 a.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the Great room (daycare classroom) in the detached guest house, LPA observed a sofa, children tables, chairs, toys in the daycare room. LPA observed kitchen area in day room and observed child proof safety latches on the cabinet where licensee stores sharp items and cleaning compounds. REPORT CONTINUES ON NEXT PAGE 1 of 4 |
