25 | REQUIRED- 1 YEAR INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced required 1- year inspection to the above facility on 07/14/23. LPA arrived at the facility at 11:30 AM and met with Lilit Antonyan, Licensee who guided analyst on a tour of the facility. A COVID 19 risk assessment was conducted prior to entering the facility. Also present during this inspection, were Aldona Avagyan and Hakob Antonayn, Licensee’s Assistants and Juleta Aristakesyan, Licensee's mother inlaw. Per licensee, Lilit Antonyan facility hours are Monday to Sunday from 12:00 a.m. to 11:55 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee.
This is a one story home located on the first level. The home consists of 3 bedrooms, 2 and 1/2 restrooms, living room, dining room, kitchen, detached garage, front yard, side yard and backyard (fenced). There is a fireplace in the living room which has been blocked off to prevent access to children. Per licensee, parents enter the facility through the side gate which is located on East Dryden Street and leads to the backyard and day care room.
Per licensee at 11:50 a.m., the children use the 1/2 bathroom in last bedroom (day care room), one bedroom (day care room), living room, dining room, kitchen, side yard and backyard fenced. Per licensee areas off limits to children and parents include: Two bedrooms, 2 bathrooms, front yard, and detached garage. LPA did not observe any stairs in the home during this inspection. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 11:50 a.m., Licensee took this LPA on a tour of the home.
