25 | On 3/18/2024 at 9:00 am an unannounced Case Management – Deficiencies inspection was conducted by Licensing Program Analyst (LPA) Carolyn Tuba who was there to conduct a separate inspection visit and during the course of the investigation separate deficiencies were discovered. LPA entered facility through the main gate and met with Staff who informed LPA that the Director, Maria Flores was currently not here. Director was called and she arrived approximately at 9:45 am. LPA took a census of 11 children in care with 2 staff.
During the visit at approximately 9:10 am. LPA discovered that a staff member was not on the Personnel Facility Clearance Report. LPA called the Department to confirm clearance and discovered there were no results of staff ever having been fingerprinted. LPA asked staff who stated that she had been fingerprinted but Director could not produce any documents that stated the staff had been fingerprint cleared. Staff was instructed to get take Live Scan Form to get fingerprints completed and cannot return to the facility until clearance has been granted. Director stepped in for staff member until additional staff could arrive. During the course of the visit LPA observed that parent dropped off child at approximately 9:15 am who is enrolled in the infant program and staff was currently with preschool age children, which is co-mingling age groups, as well as LPA observed at 11:00 am 3 school-age children were co-mingling on the playground with preschool age. LPA explained to the Director that she currently holds 3 separate licenses (Infant with toddler component, Preschool and School-age) and that children cannot be together and must remain separate during activity time both inside and outside play, as well as during rest or mealtimes. LPA provided Director with Child Care Advocate email address and discussed possible Technical Support Program (TSP) referral. LPA provided Director with Safe Sleep PIN #20-24-CCP and brochure on Safe Sleep and TSP information.
During this visit a civil penalty was assessed. All adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section
