25 | On 8/26/2024 at 12:30 pm Licensing Program Analyst (LPA) Carolyn Tuba conducted a Case Management-Annual Continuation. A COVID risk assessment was conducted. LPA met with staff member, Arely Gomez due to Director, Maria Flores was not available. LPA observed 10 children and 2 staff during this visit. The purpose of this inspection is to review staff files as they were not available for review during an annual inspection on 7/19/2024. Director was not available on that day and staff member did not know where the staff files were located at the time of the visit.
Staff Records were reviewed for completeness and found that college transcripts for 2 staff members were not available to review for teacher qualification. The health screening for 1 staff member was missing and the Health and Safety Certificate was not available in the Director’s file.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Childcare Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
The deficiency listed on the following page were observed by the LPA and is being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809-D. The deficiency that is being cited needs to be cleared to protect the children’s health & safety.
