1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 04/12/2024,at 09:00 am Licensing Program Analysts (LPAs) Shushanik Safaryan and Jessica Hopkins Hernandez ,conducted an unannounced required 3-year inspection to the above facility. A COVID risk assessment was conducted upon entry. LPAs met with Licensee, Karine Vardanian, to whom the reason for the visit was explained. Per Licensee, there are 2 children currently enrolled. Upon arrival LPAs observed only one child , second child came in later .The licensee was observed to be operating within the license capacity limitations. The licensee states that 4 adults and no children currently live in the home. LPAs did not observe A#3 fingerprints on the roster . Licensee was able to provide proof that fingerprints and LIC 508 were submitted on April 2022. Licensee will follow up with CPMB to complete the process . LPA will follow up with licensee to make sure A#3 shows under the Facility roster. This is a one story home located on the first level. There is a two entrance to the home, one door leads to the main home and one door leads to the day care area .The home consists of 3 bedrooms, 2 restrooms, living room, two kitchens, laundry room, day-care area, front yard, backyard, and side yards (fenced). Care is provided in the day-care room, kitchen in day-care area, bathroom in the day-care room, side yard (fenced). Per licensee parents enter the home through the side entrance which leads to the day care room directly. LPAs observed another front entrance door that leads directly to the off limit areas and not connected with the day care side .Only other connection from day care side to the off limit area is the sliding door which is locked during the day care hours . Per licensee, areas off limits to children and parents are living room, three bedrooms, one bathroom, 1 kitchen, laundry room, and front yard , backyard , one of the side yards .
**Rooms that are off-limits need to be made inaccessible during operating hours** During this visit, all areas identified that are accessible for children to use were inspected for the safety, comfort, and cleanliness.
At 09:20 am Licensee guided LPAs to the tour of the home. LPAs were toured starting from the entrance that led directly to the daycare room , where LPAs observed all the postings . Facility License, Emergency Disaster Plan, Parent’s Rights Poster, and Earthquake Preparedness .
Next LPAs observed small table , chairs , toys , cubbies for the children belongings . LPAs observed cots in the daycare area . Per Licensee she washes bedding after each use . Next LPA toured the kitchen where observed child proof safety latches on the cabinets where licensee stores sharp items, LPAs observed cleaning supplies stored high inaccessible to the children .Next LPA toured bathroom and did not observe anything that can pose danger to the children in care .
A current children’s roster was available for review next to the main entrance .

There is telephone service via a cellphone that stays at the facility during operation hours. Hours of operation are Monday – Friday, 07:30 am – 5:30 pm. There is central air and heating in the home. LPA Detergents, cleaning compounds, and medications were observed to be inaccessible to children. Per Licensee, there are no poisons in the home. LPA advised that any poisons should be locked under key or combination lock. At 09:40 am Licensee stated that there are no firearms or weapons stored in the home.

The valve on the required 2A 10BC fire extinguisher indicates fully charged and LPA observed receipt with a service date 04/02/2024. Smoke and carbon monoxide detectors were tested and are operable. All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Last drill documented was conducted on 02/01/2024.
The home is observed to be clean and orderly. There are first aid supplies available. There are age-appropriate toys available for children. page 2 of 5

Per licensee, she provides food for children in care. Licensee was advised that if food is brought from the children’s homes, all containers must be labeled with child’s name and properly stored . LPAs observed Child #1 food stored in the refrigerator with required labels .

Currently, children are using the backyard for outdoor play time. Per Licensee children only use the side yard which is fenced and has a grass area .The outdoor play area was observed to be fenced and gated. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA did not observe any objects that can pose danger to children. No bodies of water were observed on this day.

Licensee records were reviewed and were complete .The licensee has completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 04/23/2025.

LPA observed that the Licensee does have proof of Mandated Reporter AB 1207 compliant Child Care Training Certificate on file with expiration on 02/15/2022. www.mandatedreporterca.com
Mandated Reporter training is not provided on licensees primary language but Licensee indicated she will take the training with translation assistance and send copy of the certificate to the LPA.

Children’s records were reviewed, including emergency information and were observed to be complete. Licensee states she is not caring for any infants. LPA created an LIC 857, Children’s Record Review.

Per licensee, there is only one small dog on the premises, which is always kept in off limit areas .

Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home.
LPA did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that fall into these categories are not permitted in a family childcare facility.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at

https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA advised the licensee how to access forms, regulations and quarterly updates on the Child Care Licensing website at www.ccld.ca.gov.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California

During the exit interview, the LICENSEE Karine Vardanian confirmed that there are no Registered Sex Offenders living in the facility .

Currently, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited at this time .

A notice of site visit was given and must remain posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
