1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Licensing Program Analyst (LPA) Cynthia Reyes, conducted an unannounced annual site inspection, decrease in capacity and removal of the toddler option from the license to ensure the health and safety standards as required by the regulations governing child care centers are met. LPA met with Lydia Guerrero, Director who a copy of the Entrance Checklist for Child Care Centers form (LIC 125) was provided. A risk assessment was conducted prior to entering the facility. Due to COVID- 19 precautionary measures were taken during the entire inspection. This facility is located on the Bethany church site. The facility operates Monday-Friday, 6:30 AM to 6 PM. 1 PM, LPA began the tour of the facility with the Director Lydia, beginning at the front entrance to the facility located from the parking lot on Sunset Ave. Director states parents will enter and exit the gate and will stop at the day care office. Parents will sign the child in/out electronically using the pro-care digital system at the front office or the parents can also use a QR code on their phone to sign the child in/out and instead of taking the child to the office to sign in/out they can just take their child straight to their class room. Staff in the class room will take children temperatures and conduct health checks. LPA observed a parent board in the front office with all required licensing documents visible for everyone to see. The Directors office will be used as an ill/isolation area and has a cot/mat for a child to rest if needed. Ill children will use the staff bathroom located outside the Directors office. Licensing staff was guided through the Preschool classrooms that consist of 8 class rooms (Rooms 101/102/201/202/203/204/301 and 302.). Teacher child ratios were observed and discussed in accordance with Title 22 Regulations. Staff names and children census were recorded for this date. LPA discussed that children should be under supervision and observation, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license. Page 1 of 4

2PM Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were observed to be inaccessible to children. Lydia Guerrero, states there are no poisons, weapons or firearms on the premises.

Furniture, equipment and toys were observed and inspected to be clean, safe and age appropriate and in good repair. Telephone service, heating, lighting and ventilation were evaluated. Preschool children have their own cubby to store their personal belongings. LPA observed napping equipment/bedding for children to be in good condition, appropriate storage and cleanliness. Parents will bring child own linen. The linens will be taken home weekly on Fridays by child’s authorized representative for laundering.

Menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Menus for the past 30 days are available upon request. This facility will provide AM and PM snacks and parents can bring their child's own lunch or they have an option to purchase a hot lunch from an outside vendor (Cator Tots) used by the facility. Food preparation area is in the kitchen and snacks and lunches will be stored in the kitchen only. The licensee takes measures to keep the facility free of flies, other insects and rodents. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair.

There is drinking water available indoors and outdoors by use of pitchers and disposable cups, as well as the child own labeled zippy cup.

There is a fully equipped first aid kit available in each of the classrooms and a main large first aid kit will be located in the Directors office. If any lifesaving medication are needed they will be stored in a locked cabinet in each of the staffs own class room for that specific child.

Telephone service, heating, lighting and ventilation were evaluated. LPA observed there are fire extinguishers located right outside each class room and inside each class room are dual smoke detectors with carbon monoxide detectors that meets statutory requirements. No disinfectants, cleaning solutions, poisons and other hazardous items that are dangerous to children were observed to be accessible.

This facility will provide AM and PM snacks and parents can bring their child's own lunch or they have an option to purchase a hot lunch from an outside vendor (Cator Tots) used by the facility. Snacks and lunches will be stored in the kitchen only. The kitchen on site is clean, safe and in good repair. A monthly snack menu is available and posted. LPA advised the applicant to label and date all food, dishes and cups that are brought from home for proper storage. No transportation is being conducted by the facility. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids.

Incidental Medical Services (IMS): This facility does not plan to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. LPA observed next to preschool class room 202 is one bathroom with 3 toilets & 3 sinks and next to room 203 another bathroom with 3 toilets & 2 sinks. In room 303 is a bathroom with 3 toilets and 2 sinks. In room 101 is 1 toilet and 2 sinks. In room 102 is a bathroom with 1 toilet and 2 sinks. Total Preschool toilets 11(165) and sinks total 11. (165 Capacity)

Children's roster was reviewed for completeness. Disaster drill log was available and last drill was conducted and document on 01/20/2023. First Aid supplies were observed in each classroom and were observed to contain the required first-aid supplies. According to Lydia Guerrero, Director, medication is only administered to a child when accompanied with a doctor's note and is stored in the office. No transportation is being conducted by the facility.

Children’s Records were reviewed for completeness; Inspection of required forms were made and documented on the LIC 857. LPA reviewed staff records, which are documented on the LIC 859. Staff present have proof against TB, measles, pertussis, and influenza, as well as current Mandated reporter. LPA observed during staff file review, that there is at least one person trained in Pediatric CPR and Pediatric First Aid present during this inspection and at all time. Page 3 of 4

Carbon monoxide detectors and smoke detectors are present and are in operable condition in the facility. Smoke and Carbon Monoxide detectors should be checked, and batteries replaced as needed. LPA observed they are both located in each classroom, as well as the fire extinguisher type 2A-10BC and the valve on each indicated that they are all fully charged and each has a tag with a service date. of 02/02/2023. Fire extinguishers need to be serviced annually or as often as necessary.

Update: Effects of Lead Exposure and the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD. were discussed.

There were no deficiencies cited during today’s Annual inspection. Facility approved for removing toddler option, decrease in preschool from 169 to 154. An updated license will me emailed & mailed.
