25 | ANNUAL INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual and case management- licensee initiated inspections to the above facility on 08/08/2022. Licensee is requesting capacity increase. LPA arrived at the facility at 12:20 PM and met with Alvard Aghajanyan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, was Hayk Safaryan, Licensee’s husband/Assistant. Per licensee, Alvard Aghajanyan facility hours are Monday to Friday from 7:30 a.m. to 5:30 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee. Fire Clearance for Large Family Child Home granted on 07/22/2022.
This is a one story home located on the first level. The home consists of 3 bedrooms, 1 & 1/2 restrooms, living room, daycare classroom, kitchen, laundry room, detached converted garage that consists of a room and a bathroom, front yard, and backyard (fenced). Per licensee the children use the half bathroom in the hallway, one bedroom adjacent to the kitchen, daycare classroom, and backyard (fenced). Per licensee areas off limits to children and parents include: Living room, kitchen, two bedrooms, 1 bathroom in the master bedroom, laundry room, detached converted garage which consists of one room and one bathroom, and front yard. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 12:50 pm, the licensee began touring LPA’s through the home starting with the entry way which lead directly to the off-limit living room. LPA observed a dining table, chairs, sofas and a coffee table in off-limit living room. Per licensee, parents enter the home through the main entrance door which leads to the living room and children walk through the living room to go to the daycare classroom.
