25 | CASE MANAGEMENT INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management inspection to the above facility on 10/21/2021 due to the licensee is requesting capacity increase. LPA arrived at the facility at 12:10 PM and met with Rita Noninyan, licensee who guided analyst on a tour of the facility. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. Per licensee operation hours will be Monday to Saturday, 7:30 a.m. to 12:00 a.m.. Licensee states she cares for children 0-13 years old. Licensee was previously licensed with facility number 198020684 at 541 East Palmer Avenue, Glendale, CA 91205.
All areas identified on the facility sketch were inspected. This is two story house located on the first level. The home consists of 5 bedrooms, 2 & 1/2 restrooms, living room, playroom, kitchen, laundry room, front yard, detected garage, basement, and backyard (fenced). There is safety gate installed to the stairs in the front which leads to the street for more safety.
Per licensee children use the living room, playroom, one bedroom adjacent to the playroom, kitchen, laundry room, half bathroom in the hallway laundry room, and front part of backyard (fenced). Per licensee, parents enter the facility either through the main entrance which leads to the living room or side gate which is on Pacific Avenue and leads to the backyard. Areas off limits based on facility sketch submitted to children and parents include: Two bedrooms in the first floor, one bathroom in the hallway of bedrooms, two bedrooms and one bathroom in the second floor, basement, front yard, back of backyard, and detached garage.
At 12:30 p.m, the licensee began touring LPA’s through the home starting with the entry way which lead directly to the living room and playroom. The living room and playroom were inspected and electrical outlets
