An announced pre-licensing site inspection was conducted today by Licensing Program Analyst (LPA) Crystal Green to inspect and measure facility for capacity determination. Applicant is seeking to provide care for 75 children for a preschool program. Licensing staff met with Director of Child Development Programs, Mickie Wong, who guided analyst on a tour of the facility. Also present during this inspection was Assistant Superintendent, Richard Lohman. This proposed Preschool Program will operate from Monday through Friday from 7:30AM – 5:30PM.

LPA observed the outside of the facility to be surrounded by fencing. There is a designated pickup and drop off area for parents located on Pentland Street. Director states that drop off and pick up will be conducted at the main office. Per Director, the main entrance will remain locked until staff unlocks the door via security buzzer.

At 10:05 am, Licensing staff was guided through a tour of the proposed designated preschool classrooms. This facility will have (6) classrooms named Bumble Bee, Caterpillar, Butterfly, Lady Bug, Dragonfly, and Grasshopper. LPA observed the restroom to be located in a separate building accessible via the Main Outdoor Play area. LPA observed the preschool to have a total of 8 toilets and 9 sinks available to the children. All toilets and sinks are in safe and sanitary operating conditions. Licensing staff observed children sleeping mats located in each classroom. Individual storage space was observed available for each child inside of each classroom. Toys were observed to be clean and safe. Furniture and equipment were inspected for age appropriateness and good repair. There is a fully equipped first aid kit and fire extinguisher available in each classroom. Per Director, the isolation area for a child will be the Health Office located in the main office area and parents will be notified immediately. Report Continues Page 1 of 3

Licensing staff was guided through the proposed outdoor activity space. The outdoor area was observed to have a secured fence surrounding the property. There is adequate shade in outdoor play area available via canopy. LPA observed a bike path with bikes and age appropriate outdoor play equipment. There is a water fountain located in the outdoor play area accessible to the children. Per Director, children will have their own individual sippy cups.

Temple City Unified School District will provide snacks and lunch for the children enrolled. No transportation is being conducted by the facility.

Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA reviewed with facility representative the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Based on measurements obtained today from both indoors and outdoors, capacity for the proposed preschool program will be seventy- five (75) preschoolers. The department has received an approved Fire Clearance. A recommendation will be made to license the above facility for a capacity of 75 preschoolers.
